Abstract

The purpose of using real-time three-dimensional echocardiography of technical studies in patients with dilated cardiomyopathy left ventricular segmental myocardial synchronization index , systolic function and the relationship between the two segments . 27 normal subjects (NOR) and 24 patients with dilated cardiomyopathy non- synchronized patients (DCM) is a three-dimensional image acquisition , will be divided into the base of the left ventricular myocardium , the middle and apical three segments , measurement of left ventricular ejection fraction (LV-EF), the segmental ventricular end-diastolic volume ( REDV ) , segmental end-systolic volume (RESV), segmental ejection fraction (REF) and segmental reach the standard deviation of the time of minimum systolic volume (Tmsv-SD) and the maximum time difference (Tmsv-DIF). NOR group , the DCM group segment REDV , the RESV were increased (P lt; 0.01), each of the segments the REF were reduced ( P lt; 0.01 ) ; NOR group REF by basal segment , the middle segment , apical incremental segment , the DCM group REF according to the basal segments of the middle segment , decreasing apical segment ( P lt; 0.01 ) ; the NOR group of three segments Tmsv-SD, Tmsv-DIF pairwise comparison the difference was not statistically significance ( P gt ; 0.05) the DCM group Tmsv - SD , Tmsv the - DIF accordance basal segment , the middle segment , apical segment increments ( P lt; 0.01 ) ; the DCM group middle segment of Tmsv SD , Tmsv -DIF middle segment REF negative correlation (r = -0.6167 , -0.5992 , P lt; 0.01) ; the DCM group three segments Tmsv -SD Tmsv -DIF and LV-EF multiple regression equation the only intermediate segment Tmsv -SD Tmsv - DIF and LV-EF exists regression relationship ( β = -0.7836 , -0.7801 , P lt; 0.01) ; the three REF DCM group of three multiple regression equation in the REF and LV-EF and LV-EF there are regression relationship ( β = 0.2234,0.7506,0.1880 , P lt; 0.05), the contribution of the middle segment REF LV-EF . Conclusion DCM non- synchronization in patients with left ventricular wall systolic function and non - synchronized presence of segmental differences , not the degree of synchronization of the middle section is the main factors of the impact of global left ventricular systolic function .
