Abstract


Through years of reform, Health care system and Medical security system of China has achieved many stage achievements. Especially in 2009, "Deepening the views of medical and health system" (for short is "the new medical scheme") promulgated by the State Council build up a corresponding medical security system for town worker, urban residents, rural residents and the poorest people. This reform scheme made medical security coved almost all people, insist the policy orientation that medical institution must care more about public welfare than its own profit. This strategy needs more government financial support.This paper first gives the concept and the connotation of the medical security fiscal responsibility, then systematically investigated the change of Chinese government’s financial support and the responsibility of medical care system. Referring the various government fiscal responsibility forms of other countries’medical security system can help us summary the historical evolution of Chinese government’s fiscal responsibility for Medical security objectively. Then through looking the theory basis for the medical security fiscal responsibility, gives definitions and analysis of Chinese government’s fiscal responsibilities. The theory which include Public economics, Welfare economics, Welfare international theory, Health economics and Public finance theory, can give a perspective that the main contents of Chinese government’s medical security fiscal responsibility should include financial subsidies of Medical insurance, Medical assistance, Medical treatment at public expenses and the government subsidies on fiscal fund demands. In the basis of qualitative analysis above, the paper estimates the medical security fiscal demand of Chinese government, and then through the present condition analysis finger that there’re many realistic problems, as the unclear orientation of the government, the shortage of the Medical security’s total fiscal expenditure, lack the fiscal responsibility to Medical insurance and Medical assistance, etc. Finally, the paper gives some solutions which is Chinese government should clarity the Medical security fiscal responsibility and the specific responsibility allotment, should increase the medical security’s total fiscal expenditure, and should emphasize the fiscal responsibility to Medical insurance and Medical assistance, to ensure the fairness and efficiency of Medical security system.
