Abstract


Cerebral vascular diseases are diseases which are harmful to humanlife and heath. Ischemia cerebrovascular disease is common among themiddle-aged and the old. Its characteristics are high disease incidence rate,high prevalence rate, high recurrence rate, high mortality rate and highcrippling rate. Ischemia cerebrovascular disease is a key disease ofprevention and treatment at home and abroad. So a lot of human andmaterial resources have been devoted to the basic and clinical study of it.Some breakthrough success has been achieved at both the organic level andthe molecular level.The present research mainly deals with: dysmetabolism of neuron,microcirculatory disturbance, energy exhaustion, acidosis, the neurotoxicityof excitatory amino acid, free radical damage, inflammatory cytokines,oxidative damage, ionic imbalances, NO, necrosis and apoptosis, thedestruction of blood-brain barrier. They partly clarify the pathophysiologicmechanism of the injury of neuron after cerebral ischemia-reperfusioninjury.At the same time, the study of neurotrophic facters and neural stemcells has boosted the development of the research of ischemiacerebrovascular diseases. At present, thrombolytic therapy andneuroprotective therapy are the two intervention strategies for the treatmentof ICVD. Thrombolytic therapy is greatly limited by the time-window andprobably leads to cerebral hemorrhage, while neuroprotection has receivedmore and more recognition.To observe the impact of Kangnaoye 1 on the neurological function,cerebral infarction size and the pathomorphology of brain tissues, cerebralischemia model was made in each group for the observation of the effects of Kangnaoye 1 on Bcl-2/Bax ratio, the expression of NGF and GFAP, theeffects on apoptosis, after the focal Cerebral Ischemia-Reperfusion atdifferent times and in different positions in rats for the purpose of studingthe neuroprotection and its possible mechanismof Kangnaoye 1.Cerebral ischemia-reperfusion injury was induced by 2h of middlecerebral artery occlusion (MCAO) and followed by12h, 24h, 168h, 504hreperfusion. Two hundreds and eighty SD rats were randomly divided intoseven groups: normal group (Normal, n=40); sham-operated group(Sham-operated, n=40); model (ischemia and reperfusion) group (Model,n=40); Fasudil Hydrochloride injections group (Fasudil, n=40); Kangnaoye1 Group A (KNY 1 A, n=40); Kangnaoye 1 Group B (KNY 1 B, n=40);Kangnaoye 1 Group C (KNY 1 C, n=40). The immunohistochemistry andthe In situ hybridization were used to measure the level of Bcl-2/Bax ratio.The immunohistochemistry were used to measure the expression of NGFand GFAP, and the TUNEL and the flow cytometry was used to measurethe number level of the Neuronal Apoptosis positive cells in the cerebrumof the seven groups.Results showed that the symptoms of the injured nerves function inthe three dosage groups of Kangnaoye 1 and Fasudil Hydrochlorideinjections group have improved significantly, and the three dosage groupsof Kangnaoye 1 and Fasudil Hydrochloride injections group decreased thescore of neurological function as well as the cerebral infarction size andlessened the degree of the pathomorphological change of brain tissues; theexpression of GFAP and apoptosis increased in the necrotic area; the levelof Bcl-2/Bax ratio and the expression of NGF increased, and the expressionof GFAP and apoptosis decreased around the necrotic area (P <0.05 or P<0.01). Compared with Fasudil Hydrochloride injections group,Kangnaoye 1 Group A and Kangnaoye 1 Group B saw a decrease in thescore of neurological function, the cerebral infarction size and the degree ofthe pathomorphological change of brain tissues; the expression of GFAP and apoptosis increased in the necrotic area; the level of Bcl-2/Bax ratioand the expression of NGF increased; the expression of GFAP andapoptosis decreased around the necrotic area in the Kangnaoye 1 Group Aand Kangnaoye 1 Group B (P <0.05 or P <0.01). There were not significantdifferences between Kangnaoye 1 Group A and Kangnaoye 1 Group B(P>0.05). Compared with those in the sham-operated group, the level ofBcl-2/Bax ratio and the expression of NGF and GFAP rose, and the numberof neuronal apoptotic positive cells increased markedly in the model group.These results indicaded that Kangnaoye 1 could regulate neuronallevel of Bcl-2/Bax ratio, the expression of NGF and GFAP, and apoptosis toprotect the nerve tissue after the focal Cerebral Ischemia-Reperfusion.These changes could promote the Ischemic neuron the recovery andplasticity.
