Abstract

Objective visfatin (visfatin) or said visfatin Japanese scholars Fukuhara first reported in 2005 by the visceral fat cell secretion of cytokines, has a good insulin-like activity, in different ways to an insulin , thereby reducing blood glucose and insulin receptor binding and activation of the insulin signal channel. Some studies have shown that elevated levels of visfatin hypoglycemic effect of T2DM) patients with type 2 diabetes (type 2 diabetes mellitus. However, studies have shown that differences visfatin concentration in patients with T2DM. In recent years, many domestic and foreign literature visfatin with T2DM significant correlation, but the existence of internal visfatin with T2DM with coronary heart disease (coronary artery disease, CAD) associated rare research. By changes in the level of serum visfatin T2DM patients with coronary heart disease in the study, to understand the correlation of visfatin with T2DM with coronary heart disease, to investigate whether the level of serum visfatin as a predictor of T2DM with coronary heart disease and provide experimental basis. The method, object selection and grouping are October 7, 2007 to March 1, 2008, the People's Liberation Army 202 Hospital of Endocrinology, wards and outpatient admitted patients. T2DM 61 cases confirmed by the 1999 WHO diagnostic criteria for diabetes, a history of asking, detection and coronary angiography is divided into: ① simple T2DM group (DM) 18 patients, 11 were male and 7 females, age (45 ± 7) years of age, BMI (24.12 ± 2.45) kg/m2, blood pressure (126 ± 2) / (75 ± 7) mmHg. ② The of T2DM combined CAD group (DC) 43 patients, 26 males and 17 females, age (53 ± 8) years of age, BMI (24.58 ± 3.07) kg/m2, blood pressure (128 ± 10) / (76 ± 7) mmHg. ③ normal control group (NC) 29 cases, male 15 cases, female 14 cases, age (49 ± 8) years of age, BMI (23.13 ± 2.83) kg/m2, blood pressure (125 ± 10) / (75 ± 7) mmHg. exclude hyperthyroidism, diabetes, gout, medical history and family history. Statistically, the general information comparable among the three groups. Visfatin Human Serum visfatin: enzyme-linked immunosorbent assay (ELISA). Third, biochemical indicators of fasting plasma glucose (FPG) was measured: glucose oxidase, blood lipids: enzymatic (HDL-C, LDL-C with dextran sulfate, manganese precipitation method and formula for calculation), glycosylated hemoglobin (HbA1c) : High-pressure liquid phase method. Physiological indicators determination of the subjects was measured waist circumference, hip circumference, height, weight, ask the course of disease, age, and calculate WHR and BMI. V. Determination of the insulin resistance index HOMA-IR calculated using the homeostasis model (HOMA-MODEL) to evaluate the degree of insulin resistance, the formula is as follows. HOMA-IR = FINS (mU / L) × FPG (mmol / L) / 22.5 six coronary angiography in all patients before angiography 2d oral aspirin 100mg and clopidogrel (wave clopidogrel) 75mg. The Seldinger method from the femoral artery into the arterial sheath and injected heparin 3000U; to standard Jukins law underwent selective left and right coronary angiography confirmed coronary artery disease. Seven, Statistical analysis using SPSS11.5 statistical analysis software metering data (x ± s) that the skewed distribution data for natural logarithmically transformed for statistical analysis, t test, analysis of variance, linear correlation analysis and multiple stepwise regression. Experimental results and comparison of clinical indexes in patients with T2DM serum visfatin levels (see Table 1) simple T2DM patients with T2DM patients with coronary heart disease serum visfatin level difference (t = 4.236, P lt; 0.05); compared to normal subjects, significantly higher (t = 4.992, P lt; 0.05); the visfatin levels within the serum of patients with coronary heart disease compared with normal subjects, significantly higher (t = 4.722, P lt; 0.05). Second, the correlation analysis (Table 2) the serum visfatin content glycosylated hemoglobin, duration, BMI, TC, TG, HDL-C, LDL-C, age, HOMA-IR correlation coefficient (r = 0.744,0.042 The, 0.437,0.263,0.106, -0.411,0.437,0.089,0.491), P values ??were 0.000,0.745,0.000,0.020,0.357,0.000,0.000,0.440,0.000. Conclusions in patients with type 1 and 2 diabetes serum Visfatin concentration was significantly higher compared to the DC group and T2DM group Visfatin lower level, suggesting that levels of visfatin in T2DM patients with coronary heart disease, but the exact mechanism needs to be further explored. 2 fasting serum Visfatin level and glycated hemoglobin, BMI, the TC, LDL-C, HOMA-IR was positively correlated, fasting serum Visfatin level and HDL-C were negatively correlated, fasting serum Visfatin level and course of the disease and age no relevant sex; 3 glycated hemoglobin is the the impact fasting Visfatin level the most significant factor.
