Effects of Benfluorex Treatment on Serum Adiponectin and Leptin Level in Patients with Type 2 Diabetes Mellitus

As the development of social economics and the changes of lifestyle, the prevalence of type 2 diabetes raised rapidly, type 2 diabetes could cause many complications and threaten human’s health. As we know, insulin resistance played an important role in type 2 diabetes, so it is important to improve the insulin resistance as well as to control the blood glucose. Recent research showed that adipose tissue secreted several adipokines such as adiponectin, leptin, resistin. Adiponectin and leptin, the two adipocyte-specific plasma proteins were related with insulin resistance. Many researches showed that TZDs increased the adiponectin-mRNA expression, raised the serum adiponectin level, and inhibited the secretion of leptin, but Metformin had no such effects on adiponectin and leptin level. Benfluorex is a trifluoro methyl phenylisopropylamine compound, basic and clinical studies demonstrated that it possessed antihyperglycemic properties, improved the insulin resistance. In this paper, the effects of benfluorex on serum adiponectin and leptin level in type 2 diabetes mellitus was studied in a multicenter, randomized, double-blinded and metformin controlled clinical trial. ObjectiveTo evaluate the effects of benfluorex on serum adiponectin and leptin level of type 2 diabetes mellitus in a multicenter, randomized, double-blinded and metformin controlled clinical trial. MethodsThe patients were recuited in five hospitals. 150 patients were randomized into benfluorex group (76) or metformin group (74) for 16 weeks respectively, while 30 patients of healthy persons as the normal group. General clinical characteristics, serum APN and LEP concentration were determinated in the three groups at baseline. After 16 weeks treatment, general clinical characteristics, Serum APN and LEP concentration were determinated in benfluorex group and metformin group.Results1. Comparision of APN level in metformin group, benfluorex group and normal group: The baseline APN level in metformin group was lower than that in normal group with significant statistical significance (2.32±1.01ug/ml vs 3.46±0.83ug/ml; P<0.01). The baseline APN level in benfluorex group was lower than that in normal group (2.84±0.91ug/ml vs 3.46±0.83ug/ml; P<0.01). 16-week treatment with metformin had no effects on APN level (2.32±1.01ug/ml- 2.17±0.57ug/ml; P>0.05), and there was also no alteration of APN level in benfluorex treatment group (2.84±0.91ug/ml-2.73±0.89ug/ml; P>0.05).2. Comparision of LEP level in metformin group, benfluorex group and normal group: The difference of baseline LEP level between metformin group and normal group had no statistical significance (158.56±113.37pg/ml vs 142.77±98.62 pg/ml; P>0.05), and the comparision of LEP level between benfluorex group and normal group also had no statistical significance (149.61±110.44 pg/ml vs 142.77±98.62 pg/ml; P>0.05). The 16-week treatment with benfluorex treatment significantly increased LEP level (149.61±110.44 pg/ml-182.71±131.78 pg/ml; P<0.05) with no alteration in the metformin treated group (158.56±113.37 pg/ml-187.79±164.82 pg/ml; P>0.05). ConclusionThe serum APN level in type 2 diabetes patients is lower than that in normal persons. Metformin treatment for 16 weeks has no effects on both serum APN and LEP level. Benfluorex treatment has no effects on serum APN level, but increasing the level of LEP in type 2 diabetes patients, this suggest that the effect of Benfluorex treatment on serum LEP level is stronger than the effect of Metformin treatment.
