Abstract


The purpose of our hospital since June 2004 - December 2009 , 77 cases of upper cervical disease treatment , further analysis of the points and indications of a variety of traumatic cervical reconstruction surgery . Methods 77 patients atlas fracture 10 cases occipital fusion . Odontoid fracture odontoid screw 15 cases of odontoid fracture with atlantoaxial instability line atlantoaxial pedicle screw 8 cases . Hangman fracture line pedicle screws 8 cases , anterior C2 - 3 titanium plate 5 cases of unstable line atlantoaxial dislocation atlantoaxial pedicle screws in 12 cases, occipital fixation eight cases . C2-3 C2-3 pedicle screw dislocation line four cases , anterior titanium plate fixation in 7 cases. 31 cases of upper cervical instability according to Frankel grade : 6 cases of odontoid fracture with atlantoaxial instability E-Class , D in two cases of spinal cord injury . Of atlantoaxial dislocation unstable E-Class 5 cases of grade D four cases , three cases , C grade . 7 cases of C2-3 dislocation of the E-Class , D in four cases . Patients with postoperative the neck circumference protect from January to March . 77 patients with post-operative follow-up after 3-24 months (mean 16 months ) , X - ray film review show good reduction, in March after bone fusion . Neurological symptoms caught with varying degrees of improvement . Complications as follows : vertebral artery injury , surgery using bone wax tamponade ; atlantoaxial joint lateral sinus rupture cases , surgery to gelatin sponge hemostasis ; the dural injury cases , surgery given vascular seam line tension-free repair dura , postoperative atmospheric drainage, not caused by leakage of cerebrospinal fluid ; 1 year after surgery , occipitocervical fusion fixed patient cases of unilateral screw fracture , given the removal of implants . Based on clinical manifestations , the feel , the degree of functional recovery and radiographic assessment . Excellent in 46 cases , good in 25 cases , 6 cases , good rate of 93.2 ℅. Conclusions in the upper cervical spine surgery , the choice of surgical approach and internal fixation to injury and disease as well as the technical level decision, while it is necessary to be familiar with the characteristics of the local anatomy and biomechanics of the upper cervical spine also surgical indications should be strictly controlled , so as to safe and effective reconstruction of the stability of the upper cervical spine .
