Abstract


Background:Bile reflux gastritis is a common digestive system, has attracted widespread attention. Its pathogenesis is more complex, mainly for bile into stomach caused by the pylorus dysfunction and movement disorders of stomach and duodenum, thus causes the gastric damage.Clinically,the treatment of bile reflux gastritis is less,and there is not complete treatment,and so easy to relapse.The current treatment strategy is for its pathogenesis,on the one hand is a combination of bile to reduce the duodenal reflux material effect on gastric mucosal damage,the other is to improve the coordinationg of gastroduodenal campaign to reduce the occurrence of gastrointestinal reflux.Recently,it was discovered that the treatment combined with ursodeoxycholic acid has good effect,so now as the treatment of bile reflux gastritis based drugs.Objective: The treatment of bile reflux gastritis combined with ursodeoxycholic acid ,omeprazole,Teprenone,and Itopride,by observing the clinical symptoms (abdominal pain,bloating,nausea,bilious vomiting)response rate,the signs of endoscopy and histopathological improvement and the time of clinical symptoms disappeared to compare the evaluation of the efficacy of ursodeoxycholic acid.Methods: To February 2010to February 2011 in our hospital between the treatment,the clinical presentation,endoscopic and pathological diagnosis of bile reflux gastritis,104 patients were randomly divided into two groups.Treatment group (n=54)(ursodeoxycholic acid 250mg,qd,po;omeprazole 20mg,qd,po;Teprenone 50mg,tid,po;Itopride 5mg,tid,po.treatment for 4 weeks).The control group(n=50) (omeprazole 20mg,qd,po;Teprenone 50mg,tid,po;Itopride 5mg,tid,po.for 4 weeks).Clinical symptoms were observed response rate,the degree of bile reflux,gastric mucosal damage and histological improvement rates,while the clinical symptoms observed in each group the time spent.Count data were analyzed by chi-square test and T test to p<0.05 as significant difference.Results:1,Group of clinical symptoms (abdominal pain ,nausea, vomiting,abdominal distension)remission rates were 96.15%(50/52), 82.50% (33/40),96.00%(24/25),90.90%(40/44),significantly higher than 80.85% (38/47), 78.95%(30/38),55.00%(11/20),55.56%(20/36).Bilious vomiting symptoms of which the fastest,the treatment group(8.04+4.18 days) is better than the control group (12.18+5.38 days),with significant difference (P<0.05).2,Endoscopic signs of relief the treatment group and control group were 94.44%(51/54),78.00%(39/50).The treatment group significantly better than the control group,the difference was statistically significant (P<0.05).3,Comprehensive efficacy evaluation standard,the efficiency of the treatment group was 90.74%(49/54),the efficiency of the control group was 72.00%(36/50),the efficacy of the treatment group was significantly better than the control group (P<0.05).Conclusion:1,Ursodeoxycholic acid combined with omeprazole,Teprenone, Itopride treatment of bile reflux patients can be rapidly relieve abdominal pain, bloating,nausea,bilious vomiting and other symptoms.2,United ursodeoxycholic acid can significantly reduce bile reflux,gastric mucosal damage mitigation and histopathological changes.The integrated effect was better than omeprazole alone with Teprenone and Itopride.
