Purpose of irritable bowel syndrome (irritable bowel syndrome, IBS) is a common functional bowel disease, seriously affecting people's quality of life. So far, its pathogenesis has not been elucidated. Currently considered the pathogenesis of IBS may be related to intestinal motility disorders, abnormal visceral sensation mucosal micro-inflammation, infection, nerve - endocrine regulation abnormalities, genetic, dietary intolerance to a variety of factors such as related, including abnormal gastrointestinal motility and visceral sensation IBS incidence of major pathophysiological basis. Protease activated receptors (protease-activated receptors, PARs) belong to the G protein coupled seven transmembrane receptor family units, including four subtypes. PARs than widely distributed in the body, especially in the gastrointestinal tract, can be activated to produce a variety of biological effects, a variety of functions involved in the regulation of gastrointestinal smooth muscle, gastrointestinal feeling and mucosal inflammation, in functional gastrointestinal disorders role in the pathogenesis attracting much attention in recent years. However, most experiments are used in animal models, relatively few studies on the human body. Therefore, this study by observing protease activated receptor superfamily expression changes in the colonic mucosa of patients with IBS, to further explore the possible role of PARs in the the IBS disease process in. Research methods objects selected from the Shanghai Changhai Hospital Gastroenterology outpatient and inpatient population. (1) normal group: select 18 healthy persons as normal controls, the average age was 53.11 ± 13.65 years, 14 males, 4 females. All healthy subjects had no gastrointestinal disease history and clinical symptoms. (2) diarrhea-predominant irritable bowel syndrome (diarrhea-predominant irritable bowel syndrome, D-IBS) group: 28 cases of D-IBS patients as research subjects selected in accordance with the Rome Ⅲ diagnostic criteria, the average age of 49.044 ± 13.73 years, with male 15, and 13 were female. (3) Group: constipation-predominant irritable bowel syndrome (constipation-predominant irritable bowel syndrome, C-IB S) in accordance with the Rome Ⅲ diagnostic criteria selected for the 12 cases of C-IBS patients as research subjects, the mean age was 51.08 ± ± 10.11 years old, 5 men, 7 women. The three groups no difference in age and gender. Three groups had no significant positive signs, routine laboratory tests and laboratory examinations to exclude organic disease, colonoscopy showed no obvious abnormalities, and side forceps in the connection portion of the sigmoid colon and rectum sigmoid colon mucosa, Western blot law and real-time PCR to detect protease activated receptor superfamily expression changes. Results using the Western blot method, PAR2 and PAR4 expression in colonic mucosa of D-IBS patients with the normal group were significantly increased, its gray values ??(0.99 ± 0.67 vs 0.63 ± 0.38, P = 0.038, respectively; 0.37 ± 0.14 vs0.25 ± 0.11, P = 0.013); same time, real-time quantitative PCR results and Western blot method results are consistent (5.71 ± 2.54 vs 4.15 ± 1.87, P = 0.040; 5.51 ± 2.27 vs 3.77 ± 2.06, P = 0.031). However, the test results of the two methods showed that D-IBS patient group compared with the control group, the expression of PAR1 in colonic mucosa between the two groups was no significant difference (0.83 ± 0.27 vs 0.72 ± 0.36, P = 0.300 ; 6.70 ± 2.37 vs6.00 ± 2.65, P = 0.514). In addition, PAR2 protein PAR4 gene expression in D-IBS group and the C-IBS group was no significant difference (0.08 ± 0.07 vs 0.07 ± 0.06, P = 0.908; 3.28 ± 0.95 vs 2.14 ± 0.62, P = 0.204). Conclusion the PARs the expression, especially PAR2, PAR4, was significantly higher in the colonic mucosa of IBS patients, suggesting that it may play some role in the IBS disease occurrence, development process, and patients with certain symptoms such as abdominal pain, bloating and other close related. Therefore, PARs may become IBS new therapeutic targets.
