Objective patients with panic disorder in patients with coronary heart disease, coronary heart disease combined observation and comparative analysis of heart rate variability in patients with panic disorder to understand the changes in the autonomic nervous function theory for the treatment and prognosis of patients with coronary heart disease and panic disorder basis. And for further discussion on the relationship between panic obstacles and coronary heart disease, in order to guide clinical treatment. Methods of cardiovascular medicine inpatients in our hospital from October 2009 to July 2010, 103 cases, all selected are required to comply with angina pectoris or (and) panic disorder diagnosis and any exclusion criteria, the selected candidates were divided into three groups, are coronary heart disease group, 40 patients with panic disorder group of 36 patients, coronary heart disease with panic disorder group of 27 patients, the control group was selected over the same period to the examination of the health of our hospital volunteers, a total of 40 cases. All patients had complete line of ECG, echocardiography and coronary angiography. Using 24h ambulatory ECG monitoring system, to observe the heart rate variability sexual (HRV) of when the domain and frequency domain indexes, respectively ① All 24h within the sinus sexual RR interval period the standard difference (SDNN), units: ms; ② The 24h continuous every 5min sinus sexual RR interval average standard deviation (SDANN), Unit: ms; adjacent 24h ③ The the sinus cardiac RR interval of the difference between the share of the total number of normal cardiac the percentage (PNN50) greater than 50ms cardiac; ④ adjacent within 24h the sinus cardiac RR interval difference between the root-mean-square (RMSSD), in ms; ⑤ low frequency power (LF), the spectrum range of 0.04 to 0.15 Hz; ⑥ power high-frequency (HF), the spectral range 0.15 ~ 0.40Hz. Comparison groups using analysis of variance and pairwise comparisons between groups, the use of statistical software 17.0 for data analysis. 1, coronary heart disease group and the control group compared with lower HRV time domain indices SDNN, SDANN, PNN50, RMSSD; frequency domain indexes LF, HF reducing the difference was statistically significant (p lt; 0.05);, panic disorder group compared with the control group, the HRV time domain indices SDNN, SDANN, PNN50, RMSSD reduce; frequency domain indicators LF, HF reduce, the difference was statistically significant (p lt; 0.05); coronary heart disease with panic disorder group and the control group HRV time domain indices SDNN, SDANN, PNN50, RMSSD compared reduced; frequency domain LF, HF reduce, the difference was statistically significant (p lt; 0.05); coronary heart disease and panic disorder group and CHD group HRV time domain index SDNN, SDANN, PNN50, RMSSD decreased; frequency domain LF, HF reduce, the difference was statistically significant (p lt; 0.05); coronary heart disease with panic disorder group with panic disorder group, the HRV time domain index SDNN, SDANN, PNN50, RMSSD decreased; frequency domain LF, HF reduce, the difference was statistically significant (p lt; 0.05). Conclusion 1, in patients with coronary heart disease HRV decreased, indicating that vagus nerve function to reduce sympathetic hyperactivity, autonomic function imbalance, the occurrence of malignant arrhythmias and sudden death risk increases; 2, reduced HRV in patients with panic disorder, indicating that the vagus nerve function reduce sympathetic hyperactivity, autonomic function imbalance, increase the risk of malignant arrhythmias and sudden death occurred; 3, coronary heart disease and panic disorder in patients with coronary heart disease patients, patients with panic disorder HRV significantly decreased, indicating that the vagus nerve function reduce , sympathetic hyperactivity, autonomic function imbalance significantly; malignant arrhythmia and sudden death risk may be greater; 4, autonomic nervous system dysfunction coronary heart disease psychophysiological intermediary mechanism, with the incidence of coronary heart disease, the development and prognosis.
