Abstract


Public hospitals occupy a dominant position in China's medical services market, made a great contribution to the welfare aspects meet people's basic demand for medical services, and to promote the cause of our health. However, our public hospitals at all levels uneven development of the continuous expansion of the three medical institutions to absorb nearly 90% of the patients; level medical institutions, such as community health service centers, in apparent tilt government policies in the case, made considerable development; development strategies of the two hospitals is not clear, the medical services market is shrinking. Living in the awkward position of two hospitals, how to combine its own characteristics, to optimize the allocation of health resources, improve the efficient use of health resources, curb the occurrence of waste from the source, and has become the key to the development of their survival. Secondary care is an important part of the the Shanghai medical services system, to study how its optimal configuration, to improve its operating efficiency, the problems to be solved in the current health development in Shanghai. In this study, 20 second Central Hospital in Shanghai as the research object, the integrated use of a variety of economic and statistical evaluation of the economic performance and the production efficiency of the Shanghai Central Hospital II 1999-2009 to explore major affect hospital efficiency the degree of influence of the factors and factors, based on in-depth to explore strategies to improve the overall operating efficiency of the two hospitals to provide reference for reasonable adjustments for hospital administrators and resource input in the direction and health administrative departments to conduct macro-control. The results show that the average annual growth rate of 20 Central Hospital in Shanghai in 1999 to 2009 total revenue and total expenditure of 12.94% and 13.09%, respectively. In this study, data envelopment analysis (Data Development Analysis, DEA) based Malmquist productivity index (Malmquist Productivity Index, MPI) to measure hospital total factor productivity, to draw total factor productivity after 2006 showed low efficiency of state changes in total factor productivity is a multiple regression model, financial assistance in the proportion of total expenditure, the proportion of special equipment in fixed assets, asset-liability ratio, bed occupancy, per capita business workload due to technological progress; per patient average length of hospital stay and the number of workers the main factors affecting hospital allocative efficiency. According to the results, it is recommended that the first does not affect the asset-liability ratio to improve some of the hospitals under the premise of the overall quality of medical services, the bed occupancy rate and per capita business workload, reducing the average per patient hospitalization days, the formation of the structure of relative factor inputs optimized. Secondly, we must raise the level of hospital management, dynamic optimization of the structure of existing hospital resources according to market demand, continuously improving hospital efficiency of the allocation. Third, it is necessary to further clarify the positioning of two public hospitals, to take full advantage of the current health resources to provide a three medical institutions in different differences.
