Abstract


Purpose of this study abroad medical insurance on the basis of theoretical research on the use of health services in the fourth and Health Statistics Yearbook survey data to understand the situation of urban preschoolers basic health and medical insurance status, and according to the economic level divided selected three typical area of ??the city to do research, the focus of preschool enrollment status, health care utilization and health insurance coverage throughout, the overall level, the level of financing, payment and other issues, study of factors affecting preschool children attended medical insurance, analysis Uninsured children deep-seated reasons, to further improve our children's health care system and provide a realistic basis for policy recommendations. Methodology This paper systematically retrieve information about our children and the relevant research literature Medicare health insurance policy, through the analysis and processing for research reference. The study used quantitative research and qualitative research methods, quantitative research method is mainly questionnaires, according to China's social and economic development level, in the East, West, each extraction Suzhou, Wuhan and Guiyang City, as the areas surveyed, each survey area were randomly selected four community health centers, community health centers from each register of child care by age group were randomly selected 50 children aged 0-5, by telephone appointment on-site survey, the actual collection of 542 valid questionnaires . Questionnaire to obtain information, including demographic status, health service needs, utilization, insurance conditions and medical insurance, family economic burden; while Wuhan health insurance through a central computer system to obtain the riverbank area in 2009 the number of people hospitalized children aged 0-5, hospitalization costs and the amount of compensation and other data for analysis. Qualitative research is mainly through semi-structured questionnaire, through the health insurance sector, health administrative departments, community medical institutions to obtain key informant interviews of children enrolled in the local situation, the use of health care costs, policy formulation and implementation, and to improve and refine existing systems or the development of new policy proposals. Results (1) to establish the reality of children's health insurance system needs include child health services demand, high medical costs; urban children health insurance system imperfections; major diseases threatening the lives and health of children; building a harmonious society will inevitably requires the establishment of universal health security system, four aspects. (2) At present, China mainly children Medicare medical insurance for urban residents, the new rural cooperative medical care, hospitalization of children around the seriously ill medical insurance and medical assistance. (3) compare Suzhou, Guiyang, Wuhan City, the three children's health insurance policy are to improve children's health insurance fund-raising standards and improving the government's subsidy standards, given continuous insurance reimbursement cap line and preferential policies. Suzhou open up the insurance fund between channels; Guiyang City, the establishment of \(4) basic analysis of the survey sample, a total of 542 valid samples investigated, where the sex ratio is 1.23:1,70% mainly by children over parental care; maternal educational level mainly concentrated in the intermediate and advanced levels; three the highest average annual family income of Suzhou, Wuhan City, the lowest; 56.8% of children participate in at least participated in a medical insurance, in addition to 43.2% of children do not participate in any health insurance; children's parents do not understand is related to children's health insurance policy is not The main reason the insured; insured children with 47.08% of the children participated in medical insurance for urban residents, 24.35 percent of children participate in the new rural cooperative medical care, 25.97 percent of children in commercial insurance; parents' satisfaction with the current children's health insurance policy is only 30 %, mainly due to unsatisfactory reimbursement fewer projects, reimbursement level is low. (5) analysis of children's health services utilization and cost analysis of the prevalence of children in two weeks, 97.2% of the sick children were treated, and the illness did not seek medical treatment mainly parents think mild; 63.6% of children in the municipal hospital treatment; outpatient costs concentrated in the range of $ 100-1000. Analysis of the situation of children hospitalized, 96.4% of children requiring hospitalization due to illness were hospitalized, more than 80% of children are without referral directly to higher hospital inpatient; Suzhou City, the average cost of hospitalization of children the highest, reaching 5253.5 yuan, Guiyang City a minimum of 3,592 yuan; children suffering from pneumonia hospitalization and treatment costs are the highest. (6) Analysis of Wuhan riverbank 2009 Children's hospital medical expenses and reimbursement of expenses, the per capita cost of hospitalization was 2673.42 yuan, the per capita amount of 663.41 yuan compensation hospitalization. Conclusion gender has little effect on children insured; compared to foreign workers and non-resident population, the resident population of the city but pay little attention to children of the insured; mother's education level affects the child's participation rate; With family income improved, the incidence of the insured rising rapidly, but the average gross monthly household income level regression coefficient is negative, indicating that economic conditions are good, the more the family, the less inclined insured. Policy Recommendations (a) The Government actively guide the children to participate in medical insurance (2) encourage the establishment of children's insurance policy (3) expanding Medicare designated agency (4) for the children to participate in Medicare establish smooth flow channels and the limitations of this innovative research and innovation of: (a) the use of binary logistic regression analysis of factors affecting children attended medical insurance; (2) of the current children's health insurance policies do comparative analysis. Limitations of this study: Due to incomplete data on hospital reimbursement data is only collected, Wuhan riverbank, which is the comparison of data between different regions analyzed caused some obstacles.
