Abstract


Objectives:To evaluate the effectiveness in implantaintion of ReSTOR apodized diffractive multifocal intraocular lens.To study the difference of postoperative high-order aberration.MTF and binocular visual quality among three kinds of IOL:aspheric multifocal IOL (SN6AD3), spherical single-focal IOL(SN60AT), aspheric single-focal IOL(SN60WF).Methods:This was a prospective case series study.90 Age-related cataract patients(180 eyes)were devided into three groups with the same standard.30 were implanted bilaterally with ReSTOR apodized diffractive multifocal intraocular lens(SN6AD3), 30 were implanted bilaterally with Alcon spherical single-focal IOL(SN60AT), and 30 were implanted bilaterally with Alcon aspheric single-focal IOL(SN60WF).Ocular refraction, distance visual acuity, near visual acuity and depth of focus were observed at 3 months postoperatively. Wavefront aberration and binocular vision function.MTF were measured at 3m postoperatively by itrace. Visual quality and postoperative satisfaction were evaluated by questionaire.Results:1.UCNVA in SN6AD3 group was better than that in SN60AT group and SN60WF group;There Was no significant difference in the UCDVA态BCDVA and BCNVA among eyes with these IOLs.The mean spherical equivalent had no statistical difference between groups.2. The SN6AD3 group was better than the other two groups in depth of focus, the difference had statistical significance(F=154.511, p=0.000)3. With 3mm pupil diameter,the SN6AD3 group and SN60WF group showed a statistically reduced spherical aberration Z(4,0) comparing with the SN60AT group (p<0.05).There was no difference in Z(3,-1) (vertical coma), Z(3,+1) (horizontal coma),trefoil and total high-order aberration between the 3 IOL groups (p>0.05); With 5mm pupil diameter,the SN6AD3 group and SN60WF group showed a statistically reduced spherical aberration Z(4,0) and total high-order aberration comparing with the SN60AT group (p<0.05).There was no difference in Z(3,-1) (vertical coma), Z(3,+1) (horizontal coma) and trefoil between the 3 IOL groups (p>0.05).4. With 3mm pupil diameter, the SN6AD3 group and SN60WF group provided a statistically better MTF only in 5 cycle/degree while a statistically better MTF in all spatial frequencies with 5mm pupil diameter comparing with the SN60AT group (p<0.05).5.The SN6AD3 group had a better score in the questions of near-vision, dependence on correction and activity limitations than those in the other two groups. The SN6AD3 group had a better score in the questions of clarity of vision and diurnal fluctuations than those in SN60AT groups. Although four patients had visual glare while in the night.none of them felt discomfortable in normal lives.There is no statistical difference among three groups in glare aspect (p=0.342>0.05).Conclusions:It is a effective way to combine phacoemulsification with ReSTOR apodized diffractive multifocal intraocular lens implantation bilaterally for treatment of age-related cataract,which provided a satisfactory distance and near vision and binocular vision function.Those Clinical aspects may benefit to patients.
