Abstract


Background:The immune status of patients with gastric cancer in the early postoperative period is instability. Up to date, there is few relative research about the assess of immunity in the early postoperative period. Moreover, there is no report about the intervene in the window period with adoptive immunotherapy.Objective:To assess the lymphocyte count, cell immune function and positive、negative regulatory serum cytokines of patients with gastric cancer before or after surgery,the immune status of patients with gastric cancer and the effect of surgery on immune function were evaluated, furthermore to explore the changing regularity of immune function in perioperative period, and provide theoretical basis for immune therapy.Methods:1) The peripheral blood samples were from 34 gastric cancer patients 3 days before or 9 days after operation and 28 healthy volunteers.2) The proportion in peripheral blood of Treg cells, DC, CD8+CD28+T, CD8+CD28T cells, CD3+IFN-γ+ and CD3+IL-10+ cells were detected by flow cytometry. 3)The lymphocyte percentage and absolute count in the peripheral blood were detected; 4)The serum level of IL-12, IFN-y, IL-10, TGF-βwere detected by flow cytometry CBA technology.Results:1) The suppressor T cells (Tregs, CD8+CD28T cells, CD3+IL-10+ T cells) in peripheral blood of patients with gastric cancer increased significantly compared with the healthy control group (P<0.05), with further increase for Tregs postoperation (P<0.05); 2) The anti-tumor effector cells (CD8+CD28+T) in preoperative patients decreased significantly compared with healthy controls, but with no significant difference postoperation (P> 0.05); DC, NK decreased significantly after surgery compared with the control(P<0.05), no difference before surgery (P> 0.05); 3) The proportion of lymphocytes and absolute lymphocyte count in patients decreased significantly after surgery (P<0.05); 4) The content of inhibitory cytokine IL-10, TGF-βin patients was significantly higher than the healthy control group (P<0.05), with more increase for TGF-β postoperation (P<0.01); The content of positive regulatory T cells factor IL-12, IFN-y was significantly lower than the healthy control (P<0.05), with more decrease for IL-12 postoperation (P<0.05).Conclusions:1) The anti-tumor activity in gastric cancer patients was inhibited, and surgery lead to a more serious decrease; 2) The early period (9 days) of postoperative is the important period for adoptive transfer therapy; 3) The proportion of peripheral blood lymphocyte,NK,Treg, DC, CD8+CD28T, CD3+IL-10+in the peripheral blood of patients with gastric cancer could be used as evaluation indicators of immune function, while some serum factor such as IL-12, IFN-y, IL-10, TGF-P may be used as serum markers for assess organism immune function.
