Abstract

Objective To understand the before and after measles vaccination measles hospitalized cases, mortality, seasonal and age distribution changes, provide a basis for the development of the control and elimination of measles measures. Methods 1961-2010 50 years in Chongqing Medical University Children Hospital hospitalized aged 0-14 cases of measles clinical data, a total of 10,776 cases, stages a retrospective statistical analysis. Results With the widespread vaccination of MV, Measles hospitalized cases gradually reduced. From 20.15% in 1961-1964, the proportion of the total number of measles cases over the same period accounted for hospitalization decreased to 0.64% in 1998-2006; measles outbreaks rebounded in 2007-2008, the ratio reached 3.34% in 2007, began the measles by 2009 hospitalized cases decreased significantly. Popular non-obvious cyclical. 2. Mortality in various stages of gradual decline, down from 4.07% in 1961-1965 to 0.23% in 1998-2010. No significant difference in mortality in all ages. The male to female ratio of 1.46:1; gradual increase in the proportion of males increased from 1961-1965 1.26:1 1.80:1. 4 1961-1964 ,1965-1977 to 1998-2010, 1978-1987 from 1988 to 1997 and 1998-2010, the peak incidence: April 2 ,3-April, May and May. Measles cases in 1998 mainly in April-July, peak in May; peak in 2007 and 2008 moved from June to July. Until MV vaccination has been carried out, the measles is mainly concentrated in the 0-5 year-old children, and ≤ 8 months old and 5-14 years, the proportion of children less. MV vaccination and the implementation of planned immunization, measles incidence is still 0-5 years old children mainly, but the ratio is reduced. ≤ 8 months of age, the proportion of children in 1961-1964 ,1965-1977 1978 - 1987 1988 - 1997 1998 and 2010 were 7.51%, 6.32%, 7.41%, 32.6%, 17.36%, 32.45 %; gradually increased (P lt; 0.001). The proportion of 5-14 year-old children were 4.84%, 5.30%, 16.55%, 27.20,10.51%, increased its proportion in the EPI the beginning, but started to decrease after 1998 (P lt; 0.005). Measles hospitalized children still pneumonia (79.95%) as the main complications, followed by diarrhea (19.40%), myocardial damage (19.38%) and laryngitis (15.21%); of Encephalitis (0.83%), vitamin A deficiency disease (0.47%), and other rare. In children with pneumonia, than before to reduce the incidence increases with age. The higher concurrent diarrhea incidence of 0-2 years, laryngitis higher incidence of 0-5 years. Pneumonia, diarrhea, incidence of myocardial damage in hospitalized children with measles in 1993-2010 gradually increased. Conclusion With the application of the measles vaccine, measles hospitalized cases gradually reduced; popular cyclical obvious; With the accumulation of susceptible people, can still cause a sharp rise in cases of measles hospitalization. 2. Measles hospitalized children mortality decreased. 3 men, there is a growing trend in the proportion of children. Measles epidemic season shift. The measles Age distribution changes in ≤ 8-month-old children ratio increase. 6. Pneumonia in hospitalized children with measles complications, followed by diarrhea, myocardial damage, laryngitis.
