Abstract

The health financing and the resources assignment are the domain issues which the international society pays attention. At the beginning of 2000, the health policy department of the Organization for Economic Cooperation and Development (OECD) published system of health accounts (SHA1.0) which introduced the principles of the health accounts, the system of the indicator and the classification, and supplied a set of standard forms. OECD, the European Statistics bureau (Euro stat) as well as WHO starts to begin to carry on the revision to SHA1.0 in September,2006, the goal was to design a health account system for the world, that is SHA2.0,2010 year in September which was started to carried out in developing country,including China. As a developing country, China own 1.3 billion populations, how to satisfy everybody to enjoy the healthy health care through the limited hygienic resources is a serious challenge. The new round medicine reform began from 2009 which stressed that the government finance capacity is so important, and which is the key indicate the reforms whether the reform is successful.The research taken on Tianjin municipality city focused the health financing analysis by the SHA2.0. Under the health financing general concept, that is not only includes the fund collection, but also includes the assignment and use, that mean the paper will reflect Tianjin health fund raising process, discovered the existing questions, and supply the discussion about SHA2.0 pilot in China.The research indicated that we can see whether the allocation of government concentrated to services with high cost-effectiveness or not, as well as whether the financing structure of specific services is correspondence to their economic feature through HFXHC table. The HCXHP table can reflect that the medical and health organization main function, that is issues in provides the public health service or the medical service.HFX HP table told us the fund was allocated to the primary health institutions or high level hospitals. The SHA2.0 results can reflect the performance of the health financing, the role of the health financing scheme, especially how the government can play the role. The result of Tianjin municipality city show that the government has been playing an important role in the health financing, but the out-of pocket expenditure is also one main source. Between the different services, the government has not show the public properties, always pay attention on the curative care, and neglect the preventive care. SHA2.0 can reflect the different financing schemes,different financing sources, and how the funds flow to the different service providers, however, there are also some problems can not be solved, for example, the performance of the different financing schemes whether arrived the standard which the health financing system demands, and the intensity of the different financing sources whether reaches the reasonable ratio and so on.In short, if we want to product a reasonable methodology that fits to all over the countries, there is still a long way to go. The SHA2.0 pilot in Tianjin municipality is a useful executive. Through the pilot research, we can find the defects of the methodology, and the specificity of China when put the SHA2.0 into the research work. The pilot also open our vision in the health financing field.
