The Characteristic of Dental Implant with Flapless Surgery and the Study of Bone Resorption with CBCT

Abstract


ObjectiveThe aim of this study was to evaluate clinical effect on dental implant with minimally invasive flapless surgery,and to demonstrate the advantages and limitations of this new technique.To observe the postoperation images of CBCT and measure the thickness of bone plate around the dental implant and to confirm changes of the alveolar crest after flapless surgery.MethodsForty implants were divided into trial group (treated with flapless implant surgery using a tissue punch technique) and control group (treated with flap implant surgery). Each group had 13 patients, 20 implants placed respectively. The total surgical time of single implant placement in two groups was compared. The edema of soft tissue around implant and pain were evaluated in 1, 3, 7days after operation respectively. The shink degree of gingival around implants was measured in post-operation 2, 4, 8weeks respectively. The changes about peri-implant alveolar crest were clinically evaluated with CBCT immediately, 3 months, and 6 months post-operatively.ResultsThe average single implant surgical time was 8.55 minutes in trial group while the control group need 15.41minutes (P<0.001). The distance H was measured from the labial margin of implant abutment to the bottom of gingival border at 2-,4-,8-weeks after operation respectively.The result of trial group was 2.38mm,2.61mm,2.71mm and control group was 2.45mm,2.86mm,3.03mm.In the initial stage of implant surgery(less than 2 weeks),the result has no significantly different between trial group and control group,P>0.05.The distance H of the trial group was less than in the control group after 4 weeks of the operation, P<0.05,there is statistically significant difference,after 8 weeks of the operation such differences comes more obvious.The pain and soft tissue condition in the trial group was less than in the control group in the first week after implant placement. All patients feel comfortable and satisfactory for in trial group. As the bone thickness of the plant in lateral fall short of 1mm after flapless surgery , six months after surgery the vertical marginal bone loss of peri-implant was 0.3mm,and as the bone thickness between 1mm and 1.8mm, six months after surgery the vertical marginal bone loss of peri-implant was 0.2mm.ConclusionsThe placement of implants using minimally invasive ?apless approach compared with traditional flap dental implants, has the following advantages: (1) the surgical time is short, has the potential to reduce operative bleeding. (2) reduce the postoperative reactions, patients were significantly feeling better after surgery. (3) the surgical wound is small, reduce soft tissue damage, save the adjacent teeth shape including gum edge and gum nipple. Nevertheless,the lack of direct visibility during the flapless procedure can present risk of placement of implants with angle deviations and horizontal deviations which can lead to complications such as buccal or lingual crestal bone defects around implants.Dental implant with minimally invasive flapless surgery can effectively reduce the absorption of alveolar bone,the quantity cannot increase with the reduction of bone thickness.
