Evaluation of an Exercise and Dietary Intervention for Obesity and Overweight Migrant Children Aged 9-12 in Beijing

Abstract


Background:Obesity and overweight have become an epidemic in China, which associate with psychosocial issues and chronic diseases. What is more important, childhood obesity and overweight are more likely to become adult obesity. Studies show that the exercise and dietary intervention is important in improving the knowledge and behavior of diet and exercise and reducing BMI. But little is known about the effect in the migrant children in Beijing.Objectives:The aim of this study was to determine the effectiveness of an exercise and dietary intervention for obesity and overweight migrant children aged 9-12 in Beijing in (1) improving knowledge about controlling obesity and overweight; (2) improving self-efficiency in taking healthy food and exercising; (3) adopting healthy diet and exercise behavior; (4) reduction of BMI; and (5) to evaluate the feasibility of implementing the exercise and dietary intervention programme in migrant children in china.Method:This study adopted a quasi-experimental design.Forty seven subjects of 3-6 grades in two schools of migrant children were recruited by convenience sampling,30 of whom completed the study and compared with themselves. The knowledge, self-efficiency and behavior of diet and exercise are measured by self designed questionnaire. Data were obtained at five time points:baseline; one week after the intervention; two months after the intervention; three months after the intervention and six months after the intervention. The intervention programme consisted of eight weekly 30 minutes sessions including lecturing, providing individual dietary guidance and peer support, with the aims of improving the participants’knowledge and actively involving them in self-care management of controlling obesity and overweight, and twelve weekly 40 minutes exercises with the aims of reducing weight and cultivating the habits of exercising.Results:(1)30 migrant children’s data were collected in this study. Significant changes were found among participants in the knowledge about controlling obesity and overweight (P<0.005) after the intervention and six months after the intervention compared with the baseline; (2) Self-efficiency in taking healthy food and exercising improved significantly after the intervention (P<0.005), but there was significant increasement two months after intervention; (3) Healthy diet and exercise behavior scores got higer after the intervention and three months after the intervention compared with the baseline; (P< 0-005), and significant increasement was detected two months after intervention and six months after intervention compared with the baseline; (4) BMI reduced significantly after the intervention (P<0.005), but BMI significantly increased two months after the intervention, three months after the intervention and six months after the intervention compared with a week after the intervention (P<0.005); (4) The children were satisfied with the intervention programme.Conclusion:This study found a effect of positive changes in knowledge about controlling obesity and overweight, self-efficiency in taking healthy food and exercising, healthy diet and exercise behavior as well as BMI of participants who undertook an exercise and dietary intervention programme, but the long term effects are needed to verified. The results of the study show that the controlling of obesity and overweight is a long term process which needs the cooperation and supervision of the parents. The study was effective, and it is possible to carry out the intervention programme and worthy to offer such programme to more migrant children.
