Abstract

Since the founding of New China,especially since the reform and opening up,great achievements have been made in our hygienic industry which aims at defending the rights and interests of people’s health and improving people’s healthy standards during the whole process of social and economic development . The major health indicators are among the front ranks of developing countries and have reached the average level of those middle and high-income countries.While during the fast development of the medical and health industry,the gaps in the medical and health services between city and country are becoming more and more apparent,which is manifested on the inequality of the medical and health services enjoyed by people between city and country.However ,the fundamental reason under this issue is the unbalanced assignment of health resources between city and country. Recent years our government have come up with a series of reforming issues on the equalization of public services as well as the scientific outlook based on coordinating the development between city and country.The new medical reform program has also required that the primary care systems should be further completed and developed between city,town and village,besides,the basic health care system enjoyed by everyone should be established in order to sustain the fairness and justness of medical health.The fact is that health resources are the foundation of health development ,and the balanced assignment of health resources directly relates with output of health service,whose fairness is key to the fairness of medical and health service.The equalization of health resources allocation between city and country is the key to basic health care system,this thesis base on the equalization of health resources between city and country,analyze the situation and the reason , then offer some policy proposals to make some contributions to realizing the fairness of the basic medical and health service between city and country.This thesis will start from the Theory of Public Goods and Equality Theory,that is to say,the basic medical and health service includes the basic medical and public health service ,with the nature of public goods , which should be equally offered to everyone residents.Moreover,the equalization of health resources reflects the maintenance of basic health rights , with the inner nature of fairness.Then,in the description of the historical evolution of the health resources,the thesis analyzes the current situation of the assignment of health resources between city and country from several aspects such as the input of medical fund,medical manpower,and medical material resources,which point out the unfairness in medical health services,the disparity in health lever,and the difficulty and expensiveness in medical treatment . At the same time , the paper analyzes the disequilibrium of urban and rural health resources allocation from Changshu City at micro level.Then the thesis summarizes and analyzes the practices of ChangShu in the promotion of urban and rural health resources equalization in recent years.There are many reasons leading to the unequal allocation of health resources between city and country,though, this thesis will analyze from macro aspects to micro ones.The policies of concentrating on city whiling neglecting town and laying stress on industry at the expense of agriculture under the policy of the dual economic structure of rural and urban areas have created a tendency to city.The medical system has been marketed during our economic transition to market economic system and the price determines resources allocation which made market failure.What’s more,there exists a contradict between wealth authority and affair authority of the local government after the tax-sharing system,and financial difficulties among the towns and countries governments has seriously affected input in health industry . The rigid health administration has also constrained the optimum of health resources.Finally,on the basis of the former reasoning,corresponding policies and measures have been made.We should integrate the coordination of rural and urban development and hold to governmental dominant resource allocation.We should further improve the financial system and long-effective mechanism for the rural health input should be established.We should reform the system of health administration and human resource management to facilitate rational flow of health resources.
