The Pilot Study of Correlation between Serum Estradiol Levels of Premature Infants and the Incident of the HMD and BPD

Abstract


Objective:We detected serum levels of estradiol during early postnatal life to investigate the relationship between the estradiol and development of the fetal lung and the incident of the pulmonary hyalinemembrane disease(HMD) and the bronchopulmonary dysplasia(BPD).Methods:Fifty-nine premature infants whose gestational age is from 26 weeks to 32 weeks were studied.We identified the HMD or BPD as the evaluation of the development of fetal lung.Sixty-one full-term infants whose gestational age between 37 weeks and 42 weeks were studied as control group.We tested the levels of estradiol in serum on the 1st,3rd and 7th day after birth using the methods of competitive immunoassay(ADVIA).The statistic analysis was completed by the software of SPSS 13.0.Results:(1) The serum levels of estradiol decreased gradually on the 1st,3rd and 7th day after birth and have significantly difference(F is 56.59 and 5.65;P is 0.000 and 0.0009,respectively ) in both the premature and full term infants.(2) There were no significant difference(t is 0.476,1.329 and 1.829;P is 0.441,0.187 and 0.072, respectively) of the estradiol levels in the group of preterm and full-term infants on the 1st,3rd and 7th day after birth.(3)On the 1st,3rd and 7th day after birth there were no distinct difference(t is 0.777,1.718 and 1.829,whereas P is 0.441,0.092 and 0.064, respectively ) of serum estradiol in preterm infants with or without the HMD.The difference of the estradiol between the premature with HMD and full-term baby were no significant difference on the 1st day(t=0.564,P=0.575 ),but on the 3rd and 7th day after birth,the estradiol in the serum markedly increased(t is 2.107,2.604 and P is 0.039 and 0.011,respectively) in the premature infants.(4) There were no remarkable difference(t is 0.045、9.878,P is 0.769、0.07,respectively) of the estradiol levels in premature inthnts with or without the BPD on the 1st and 7th day,but on the 3rd day after birth,the difference was distinct which patients with BPD were very higher than those without the BPD(t=0.443,P=0.660 ).Moreover,we compared the full-term infants with BPD and found the levels of the estradiol were no prominent difference (t=0.443,P=0.660) in both objects on the 1st day after birth,but on the 3rd and 7th day after birth the difference is very marked(t is 2.427、2.60,P is 0.018、0.011, respectively),and serum estradiol levels of the patients with the BPD is higher than full-term infants.Conclusions:(1) the levels of serum estradiol decreased gradually on the 1st, 3rd and 7th day after birth in both premature and full-term infants and even declined to the zero on the 7th day after the birth(premature:4.62±12.50pg/ml;full-term infants:1.31±4.04pg/ml) which manifested that estradiol levels decreased more rapidly in full- term than in premature infants.(2)the levels of estradiol were no association with the HMD and BPD on the early postnatal period,which implied the detection of estradiol were no help to predict the incident of HMD and BPD in the newborn.(3) There were no sexual difference of estradiol levels in early neonatal life.
