Abstract


Objective:We want to explore the pathogenesis and treatment of Dupuytren’s contracture.We hope to find the best type of operation corresponding to Huang clan grouping typing and the influse of the Risk factors in this disease.Methods:A total of 80 hands of 60 cases of Dupuytren’s contracture from June 2000 to February 2009 were studied retrospectively.That all undertaked partial resection of palmar fascia, the entire resection of palmar fascia and so on.Pathological examination was performed in 9 cases.Result:All cases were followed up for an average of 4 years,most cases were freed from contracture of palmar aponeurosis and recurrence were found only in 26 cases. Conservative treatment of one of the excellent rate of 25%, subcutaneous excision Dupuytren 33.3%, partial laryngectomy Dupuytren 50% ,Dupuytren hysterectomy 50%.Subcutaneous hemorrhage,and Incisal opening delayed healing on 28 hands.Conclusion:In the basis of Huang clan grouping typing,relatively rare in China.Northeast patients’clinical classification is usually in HuangⅢmore than HuangⅠ,Ⅱ.The operation of HuangⅡwas better in partial resection of palmar fascia. HuangⅢwhich used Dupuytren hysterectomy is more positive effect,and it’s satisfactory and low relapse rate. In addition, Dupuytren hysterectomy is also better for HuangⅣ.According to pathology and clinical manifestations it is not difficult to speculate subcutaneous nodular fasciitis is one type of early manifestation of Dupuytren’s contracture. At the same time, some of the basis of disease and Dupuytren’s disease risk factors will also affect the prognosis of this disease so that the possibility of skin necrosis, wound healing complications such as extension.
