Abstract


Objective: pulmonary thromboembolism is a complication of malignant tumor incidence of pulmonary embolism increased significantly in cancer patients than in the general population .Even thrombosis is probably one of the early performance of cancer, maybe the potential signal cancer. Malignant tumor with thrombus will not only increases the difficulty of treatmentm,but also make the patient’s quality of life,and short patients live.Thromboembolism has become the second leading cause of death in cancer patients. In recent years ,the association of malignant tumor and thromboembolism has caused wide public concern. Blood high pourpoint state is the most important reasons for tumor with pulmonary embolism, and surgery, chemotherapy treatment such as measures to increase the risk of tumors occur of pulmonary embolism. Age as an independent risk factors involved in the progress of tumor concurrent pulmonary embolism.In this paper,retrospectively analyzed nearly seven years of 40 cases of cancer patients with pulmonary embolism clinical data, discuss of malignant tumor complicated with pulmonary embolism in the general clinical features,risk factors,laboratory findings,diagnosis,treatment and prognosis.Methods: Retrospective ananlysis clinical data of the Fist Affliated Hospital of Dalian Medical University,7-years period 40 cases of malignant tumors with pulmonary embolism,and to collect the same period 40 cases of pulmonary embolism is not associated with malignant tumours in patients with such information as the control group.Discuss of malignant tumor complicated with pulm- onary embolism in the general clinical features,risk factors,laboratory findings, diagnosis,teratment and prognosis.With help of software SPSS 17.0,we used Logistic regression to evaluate the risk factors ,and Measurement data using T-tests testing method ,cont data using X2 test.P<0.05 statistically means. Results: 40 patients in case group, the male 25 cases (60%), women 15 cases (40%), and the control group male 21 patients (53.3%), women 19 patients (46.7%),Case group and control group gender was not significant (P values were 0.26 and 0.136) . Adenocarcinoma is most of the pathologic types of tumors,a total of 40 patients in case group ,in which of 21 case of adenocarcinoma ,accounts for the case group 52.5%. Two groups in the type of pulmonary embolism are mainly in the area of pulmonary embolism,different types of PTE in the two groups no significant difference (p=0.143).We fond that many factors ,such as surgery、immobilization and age can promote the cancer patients to concurrence with thrombosis diseas. Both groups have different degrees of hypoxia, upon examination, two groups of material was not statistically significant.Testing the two groups of APTT, TT, PT and D-dimer there were no statistically significant. two groups of Fib with a statistical significance, Results The total effective rate of early anti-coagulated and thrombosis-dissolved treatment is 67.5% in the case group, mortality is 27.5%,and in the control group the effective rate is 90% and mortality is 7.5%. The 68 patients treated, more than half of the patients considerable improvement in 3 days.Conclusion: 1.Blood coagulation state happens in patients diagnosed malignant tumor complicated with surgery and immobilization, which has higher risk level of pulmonary thrombosis embolism compared with the control group. 2.The age has relationship with the pathogenesis of pulmonary embolism process. It is suggested that high-risk patients should take preventive anticoagulation actively in order to improve the prognosis.
