Abstract


Objective: To compare the clinical features and pulmonary manifestations of mycoplasma pneumonia in children of different age groups.Methods: Collected the clinical data and ancillary test results of 102 children diagnosed with mycoplasma pneumonia from June 2009 to June2011,in Dalin Central Hospital.they were retrospectively analyzed and summarized.All children were divided in to two groups: infant group (0 to 3 years old) and older children group (3~14years).Compared the differences of clinical symptoms、signs、X-ray、laboratory and pulmonary manifestations. Chest radiographs were taken of all children.we censorship serum IgM antibodies submission,peripheral blood white cell count, urine routine,CRP,enzymes,liver function and other conventional checks on the next day morning at the hospitical and selective for ECG, EEG, ultrasound and other related checks. Records of each patients age, gender, admission date, clinical symptoms, signs, pulmonary manifestations, treatment programs, were analyzed. Statistics statistical software SPSS16.0 credit processing application software package, using X 2 test to P <0.05 as statistically significant difference.Results:1. Comparison of the main symptoms and sings the rates of low heat,medium degree of heat, high heat were 21.7% ,34.8%, 10.9% in Infant group.older children group was 7.1%、35.7%、26.8% respectively.the rates of cough, wet rales, wheezing were 100%, 84.8%, 34.8% in Infant group. the rates of older children group were 85.7%, 30.4%, 8.9%.the differences of Two groups with fever, high fever, cough, wet rales, wheezing in incidence was statistically significance (Pall<0.05); incidence of moderate degree heat was no significant difference (P>0.05).2. Chest X-ray comparison of two groups the rates of invasive lobular real, substantial segments of lung infiltration, interstitial infiltration in infant group were 71.7%, 13.0%, 15.2%; older children,group rates were 8.9 %, 87.5%, 3.6%. Two groups of children with chest X-ray changes were statistically significant differences in comparison (Pall<0.05).3. Laboratory test results C-reactive protein was 17.4% increased in infant group, 37.5% increased in older group, CRP increased rate of the two groups was statistically significant (P<0.05),CRP increased in infant group was higher than older children group4. Comparison of two groups of children with pulmonary complications. in infant group,the rates of cardiovascular involvement ,digestive symptoms, liver damage, nervous system involvement, blood system involvement, skin lesions, renal injury rates were 19.6% ,34.8%, 47.8 % 2.2%, 6.5%, 2.2%, 4.3%. the rates of Older children group were 44.6%, 14.3%, 21.4%, 10.7%, 5.4%, 5.4%, 10.7%. The differences of two groups in the cardiovascular, liver, digestive system, nervous system, kidney damage areas of pulmonary complications was statistically significant (Pall<0.05); in the skin, blood-pulmonary complications in terms of no significant significance (Pall> 0.05).5. Comparison of the cure rate in two groups the cure rate of the older children group was 96.4%,infant group was97.8%,no significant difference in cure rate. (P <0.05)Conclusion:1. low fever was more common in infant group and many accompanied by breathing.wheezing,wet rales and other signs of multiple lung were more than older children group. high fever was more common in older children group ,and more without breathing, wet rales, wheezing signs such as the lungs were less than infant group.2. the chest X ray of infant group mainly showed the invasive lobular disease, older children group mainly showed the pulmonary infiltrates substantive paragraph.3. CRP increased rate of older children group was significantly higher than infant group.4. infant group take the liver, gastrointestinal symptoms as the main performance in pulmonary damage; older children group, cardiovascular involvement was more common in older children group, extrapulmonary manifestations were more serious than infant group.
