Abstract


Objective To investigate the normal prostate, benign prostatic hyperplasia (BPH) and prostate cancer (PCa) Diffusion-weighted MR imaging (DWI) and apparent diffusion coefficient (ADC) map the performance of the ADC value differential diagnosis of PCa. Materials and Methods 15 cases of normal prostate, BPH 48 cases,, PCA 28 cases and prostate tuberculosis patients for routine MRI scan and MR diffusion-weighted imaging, analysis DWI and ADC map performance. Measuring different b values ??(b = 400,500,700,800,1000 s / mm 2 ) PCa, BPH and normal prostate the central glands (CG) and the outer peripheral zone (PZ) ADC value, as well as prostate tuberculosis ADC value and ADC value of simultaneous measurement of bladder area. ADC values ??of the normal group, the group with BPH and PCa group for statistical analysis, comparison during which the difference is statistically significant. DWI and ADC maps clearly show the center of the normal prostate gland and peripheral zone; BPH the peripheral band signal on DWI and ADC maps more uniform, central gland signal due to uniform; b = 700s/mm 2 , the PCA DWT Figure performance of high signal, low signal on ADC Figure. Prostate tuberculosis cases in the DWI map prostate signal due to uniform gland lesions were the round slightly lower signal or slightly hyperintense lesions showed low signal in the corresponding ADC map, the ADC value decreased, mean ADC value: (0.659 ± 0.102) × 10 -3 mm 2 / s. b values ??are the same, bladder ADC value among the groups was no significant difference. 15 cases of normal prostate peripheral zone and central gland mean ADC values ??were as follows: (1.884 ± 0.378) × 10 -3 mm 2 / s and (1.602 ± 0.275) 10 -3 the mm 2 / s; 48 patients with benign prostatic hyperplasia outer peripheral zone and central gland area hyperplastic nodules mean ADC values ??were: (1.762 ± 0.170) × 10 -3 mm 2 / s (1.485 ± 0.351) × 10 -3 mm 2 / s ; 28 cases of prostate cancer Ca stove and central gland tumor area mean ADC values ??were: (1.210 ± 0.217) × 10 -3 m m 2 / s (1.467 ± 0.321) × 10 -3 m m 2 / s. PCa foci ADC value was significantly lower than that in normal prostate, BPH hyperplasia, during which the ADC value difference was statistically significant difference between the PCA group and normal prostate group group and PCa group was statistically significant difference between the group and the BPH group (F = 67.821 , P = 0.00), no significant difference between the normal prostate group and BPH group (P = 0.341). Conclusion (1) MR diffusion-weighted imaging can be used for the examination of the prostate, can reflect the pathological changes of the lesions, and provide quantitative diagnostic information. (2) diffusion-weighted MR imaging of PCa important diagnostic value the DWI figure clearly shows the position of PCA, scope, PCA lesions showed high signal on DWI Figure, the ADC value was significantly lower than that in normal tissue and BPH hyperplastic tissue, can be used for differential diagnosis and early diagnosis of PCa. (3) conventional MRI combined with DWI, on the basis of conventional MRI morphology, combined with the pathological changes of the signal characteristics of the lesions on DWI Figure ADC value information, can improve the diagnosis of PCa, improve diagnostic accuracy.
