Objective: To investigate the heparanase (Heparanase, Hpa) expression in renal tissues of patients with chronic kidney disease distribution and its relationship with the clinical manifestations. Method: The expression levels of non-biotin two-step immunohistochemical SABC method detected 36 cases of patients with chronic kidney disease, glomerular and tubular interstitial heparanase (Heparanase, Hpa), and combined with clinical data and kidneys pathology were analyzed statistically. The results: 1. Of heparanase (Heparanase, Hpa) in the control group of normal renal tissue glomerular basement membrane, glomerular podocytes, mesangial cells and tubular epithelial cells with little expression. Lesions in the renal tissue of patients with chronic kidney disease, Hpa expression was significantly enhanced, compared with the normal control group, the glomerular and tubular epithelial cells in the renal tissues of patients with chronic kidney disease have Hpa expression, the expression level of the control group significantly higher compared with a statistically significant difference (P <0.01). Compared with the control group, membranous nephropathy group, MN glomeruli and. Tubular epithelial cells Hpa expression levels were significantly increased, with statistically significant differences (P <0.01). Of I MN group glomerular and tubular epithelial cells Hpa expression levels were significantly increased compared with the control group, a statistically significant difference (P <0.01). Phase II MN group glomerular and tubular epithelial cells Hpa expression levels were significantly increased compared with the control group, a statistically significant difference (P <0.01). Ⅰ of the MN group II period MN group compared glomerular and tubular epithelial cells Hpa expression difference was not statistically significant (p> 0.05). 3. The glomerulonephritis group glomerular and tubular epithelial cells Hpa expression increased, compared with the control group, a statistically significant difference (p <0.01). Mild, moderate and severe mesangial proliferative group with mesangial cell proliferation degree aggravated, Hpa expression in glomerular and tubular epithelial cells gradually increased, severe mesangial proliferative group with moderate mesangial proliferative group, moderate mesangial proliferative statistically significant difference (p <0.05) compared to the group with mild mesangial proliferative group glomerular and tubular epithelial cells express mild mesangial proliferative, severe mesangial proliferative group the group there was a significant glomerular expression difference (p <0.01)) .4. sclerosing nephritis glomerular and tubular epithelial cells Hpa expression increased, compared with the control group, the difference was statistically significant (p <0.01). 5. Pathological type group comparison no statistical significance (p> 0.05) 6.Hpa and clinical indicators of correlation between the expression of glomerular and tubular epithelial cells ttPA area and the average gray: Hpa 24-hour urinary protein excretion was positively correlated, r = 0.541, (p <0.01). Statistically significant; is negatively correlated with serum albumin (r = -0.694), (p <0.01), a statistically significant;, lipids, and serum creatinine immunoglobulin correlation. Conclusion: heparanase (Hpa) glomerular basement membrane in normal kidney tissue, glomerular podocytes and mesangial cells and tubular epithelial cells with little expression. Glomerular basement membrane disease group, glomerular podocytes, Department of mesangial cells and tubular epithelial cells express increased significantly. Glomerular and tubular epithelial cells of 2.MN patients compared with the control group, the expression of Hpa significantly enhanced, consistent with the MN urine protein increased, suggesting that the abnormal expression of Hpa involved in the formation of the IMN kidney damage with proteinuria. Plays an important role. Glomerulonephritis group with the aggravation of the severity of Hpa expression was significantly higher. Glomerular and tubular epithelial cells Hpa expression levels and activity progress of nephropathy. 4.Hpa expression of the degree of proteinuria positively correlated, suggesting that the enzyme Hpa as a proteinuria is closely related, have been more and more attention has been paid, will become proteinuria treatment target.
