Abstract


Background Mechanical ventilation has been widely used clinically critical care medicine, and other fields, is an important means of rescue. Mechanical ventilation in patients with a high incidence of malnutrition has become common in ICU potentially fatal complications, but also led to one of the main ventilator dependent. Mechanically ventilated patients with high metabolic reactions early multi-characterized clinical manifestations of catabolism, body proteins and lipids consume a lot of memory, respiratory muscle atrophy, respiratory weakness, ventilator dependent. So how to critically ill mechanically ventilated patients in nutritional support is essential. Aim of the present study was to investigate the Chinese Nutrition (Sijunzitang) for critically ill patients with mechanical ventilation nutritional support clinical effect, through prospective randomized study comparing add Sijunzitang enteral nutrition (EN) support a variety of nutrition indicators before and after the change , changes in respiratory function and adverse reactions to medicine in the field of nutritional support in critically ill application made some attempts. Methods from April 2007 to February 2009 Check our ICU mechanically ventilated patients, 50 cases were randomly divided into treatment group and control group 25 cases, collecting clinical data. Control group received conventional enteral nutrition support program (Nutrison 500ml), the treatment group received conventional nutritional support (Nutrison 500ml) plus Sijunzitang 200ml. Both in support and nutritional support before the tenth day after the monitoring nutrition indicators such as serum albumin (ALB), prealbumin (PA), transferrin (TRF), hemoglobin (Hb), cholesterol (TC), triglycerides ( TG), triceps skinfold thickness (TSF), arm muscle circumference (AMC), lymphocyte count (TLC) and so on. And observe their reactions and complications of malnutrition, the number of days of mechanical ventilation and so on. Measurement data using t test, count data using chi-square test, P <0.05 was considered statistically significant, all statistics were performed using SPSS 13.0 statistical software. Results ① treatment 10 days after the pre-albumin, transferrin were improved in the treatment group improved more significantly between the two groups were significantly different (P <0.05), there are statistically significant. ② research shows albumin, arm muscle circumference, triceps skin fold thickness in the two groups showed no significant change in EN support, no statistical significance (P> 0.05). ③ treatment group improved respiratory function better than the control group, the number of days of mechanical ventilation was significantly less than the control group, the difference was statistically significant (P <0.05). Conclusion add TCM Sijunzitang enteral nutrition agent enteral nutrition agent than simply better to improve the nutritional status of patients with mechanical ventilation, 10 days after treatment nutrition indicators such as albumin, prealbumin, transferrin and hemoglobin in both l high, the treatment group improved more significantly, compared with the control group, the difference was statistically significant (P <0.05), and can improve the patient's respiratory function and shorten duration of mechanical ventilation. Medicine confirmed nutritional support in critically ill certainly has some potential applications.
