Abstract


Purpose of osteoarthritis (osteoarthritis, OA) is a common orthopedic diseases. Accompanied by the increase of the aging population in the world in recent years, the the OA incidence also showed an increasing trend. OA exact cause is unknown, there are diverse views, including matrix metalloproteinase family of matrix metalloproteinases (MMPS) and its inhibitor family of tissue inhibitor of matrix metalloproteinases (Tissue Inhibitor of Metalloproteinase in) with OA relations have become increasingly subject to the attention of the medical profession. Articular cartilage degeneration has always been considered to be a sign of OA. The current study shows that osteoarthritis is due to the imbalance in the articular cartilage extracellular matrix synthesis and degradation of MMPs / TIMPs imbalance is one of the main reasons that cause cartilage degeneration. We selected matrix metalloproteinases and their inhibitors of matrix metalloproteinase family -7 (matrix metalloproteinase-7, MMP-7), matrix metalloproteinase -9 (matrixmetalloproteinase-9, MMP-9), tissue inhibitor of matrix metalloproteinase -3 (tissue inhibitor ofmatrix metalloproteinase-3, TIMP-3) using immunohistochemical method combined cell research in osteoarthritis the synovial expression, thereby strengthening early diagnosis and early treatment of such patients. Methods 60 patients with knee joint synovial specimens application SABC immunohistochemical method color matrix metalloproteinases MMP-7, MMP-9 and its inhibitor of metalloproteinase inhibitor of TIMP-3 expression in lesions and normal groups, and with HE staining cells was observed. Clinical data hyperplasia and diseased synovium in the surgical treatment of patients hospitalized in our department from November 16, 2007 to 11 October 2008, all cases were diagnosed by the hospital's orthopedic and imaging. OA patients with a total of 30 cases, the lesion group, aged between 48 to 75 years old, arthroscopic exploration debridement, intraoperative specimens from the synovial membrane. Non-OA group, the control group also thirty cases. Synovial folds syndrome in 4 cases, meniscus injury 20 cases before and after cruciate ligament injury in 6 cases, arthroscopic surgery take into road clean-up of the synovium. Second, within research methods postoperative 1h, synovial flushed with saline, cut into 1.0cm × 0.8cm × 0.5cm small pieces. With 10% neutral poly formalin-fixed, paraffin-embedded, 5μm slice. Disease group and the control group were randomly selected two cases of synovial HE staining, light microscope observation of the morphology of the synovial cells. The remaining sections were immunohistochemical SABC staining, observed under the microscope stained sections eyepiece grid micrometer optional 10 vision (400X), positive cell count of 100 cells per field, mean ± standard deviation. Matrix metalloproteinases MMP-7, MMP-9 and its inhibitor of metalloproteinase inhibitor TIMP-3 expression in normal synovial expression in patients with severe osteoarthritis of the knee synovium significant difference (p lt; 0.05). First, the results of histological results show no chronic inflammation in normal synovial tissue, synovial cell distribution rules, no obvious proliferation and capillary structure and no significant pathological changes in the surrounding tissue, and OA synovium was chronic inflammatory response , a large number of lymphocytes and mononuclear cell infiltration, and around the blood vessel distribution, synovial cell hyperplasia, showed focal line papillary or villous hyperplasia, capillary proliferation significantly monolayer of vascular endothelial cell swelling, small vessel lumen narrowing peripheral vascular fibrosis and deep fibrosis, fluff, focal and peri-vascular glass-like change, stromal hyalinization, cartilage metaplasia. Second, the immunohistochemistry result MMP-7, MMP-9, TIMP-3 in OA synovial tissue, synovial cells, monocytes and lymphocytes, vascular endothelial cells and metaplasia of cartilage cells positive for expression cytoplasm brownish yellow, while relatively little expression in the normal synovial tissue. But the overall statistical results showed that the rate of lesions positive cells compared with control group significant difference, P lt; 0.05. Conclusion matrix metalloproteinases MMP-7, MMP-9 and its inhibitor of metalloproteinase inhibitor TIMP-3 expression is an important symbol of osteoarthritis diagnostic indicators.
