Abstract


Objective: To detect the Duffy antigen chemotactic factor receptor (Dufffy the antigen receptor forchemokines DARC), the cytoskeletal protein Fascin expression in laryngeal squamous cell carcinoma and analyze its relationship with clinicopathological factors of patients with laryngeal cancer, prognosis explore its clinical significance. Methods: 65 patients were divided into lymph node metastasis and non-metastasis group DARC and Fascin expression in laryngeal carcinoma were detected by immunohistochemistry method, simultaneous detection of laryngeal cancer tissue and adjacent normal larynx tissue ( away from the edge of the tumor> 0.8cm, and infiltration of tumor cells) tumor microvessel density (microvesseldensity, MVD); addition to detect normal larynx tissue next to each cancer case patients pathologically positive lymph node metastasis and pathological negative lymph nodes Fascin expression. Results: 65 patients DARC strong expression in 32 cases, accounting for 49.2%; the DARC weak expression of the 33 cases, accounting for 50.8%; lymph node metastasis group and non-transfer group DARC strong expression of 8 cases (25%) and 24 patients (72.7%, respectively. ), the difference was significant (P = 0.000). The DARC strong expression in 32 cases of 33 patients with microvascular density (MVD) were (24.75 ± 5.45) / HP (32.58 ± 5.23) / HP, the difference was significant (P = 0.000) and weakly expressed. DARC expression of the degree of MVD, lymph node metastasis, clinical stage, local tumor recurrence and distant metastasis was a significant negative correlation (correlation coefficient r = -0.612, -0.477, -0.419, -0.320). In laryngeal carcinoma cancer and adjacent normal tissue, Fascin expression difference was statistically significant (Z = -6.000, P = 0.000). The Fascin strong expression of lymph node metastasis and non-metastasis group were 23 cases (71.9%) and 13 patients (39.4%), the difference was significant (P = 0.008). 32 cases of patients with lymph node metastasis, the primary tumor and the metastases of the same patient Fascin expression, the primary foci Fascin strong expression was 65.6% (21/32), and the lymph nodes the Fascin strong expression was 40.6% (13 / 32), the expression of both the difference was not statistically significant (t = 0.128, P = 0.066). Laryngeal carcinoma Fascin strong expression and T stage, lymph node metastasis, local recurrence and distant metastasis, tumor type, tumor size, the difference was statistically significant (P <0.05). Single factor analysis of the survival rate of the results showed that the the the DARC strong expression of survival time are related, and the the Fascin strong expression of negative correlation with survival time. To analyze the survival time of patients with laryngeal cancer, DARC strong expression the patients, and DARC weak expression in patients with average survival times were 74.05 and 58.27 months, two groups of survival rate difference was statistically significant (P = 0.021). Fascin strong expression, and the average survival time for patients with weak expression were 56.59 and 84.22 months, the difference was statistically significant (P = 0.026). The correlation analysis showed laryngeal carcinoma DARC Fascin expression between the difference was not statistically significant (P = 0.398, correlation coefficient r = -0.107). In order to determine the independent prognostic factors in this group of patients, using the Cox proportional hazards model for multivariate analysis, all the univariate analysis the factors included in the model appears, Backward LR method gradually remove non-independent prognostic factors for the conclusion that four independent prognostic factors: of Fascin (P = 0.04; HR = 10.043; 95% confidence interval ,1.106-91 .163), T staging (P = 0.049; HR = 0.424; 95% confidence interval ,0.180-0.998), neck lymph node metastasis (P = 0.012; HR = 3.664; 95% confidence interval ,1.331-10.084); local recurrence and distant metastasis (P = 0.001; HR = 46.928; 95% confidence interval ,5.123-429.839). Conclusion: 1.DARC in laryngeal reduced expression and MVD increase may be susceptible to tumor metastasis to the lymph nodes and other distant sites, lymph node metastasis, can be used as predictors. 2.DARC by reducing tumor MVD inhibit tumor formation and metastasis of laryngeal cancer. The laryngeal cancer invasion and metastasis in 3.Fascin plays an important role in promoting Originally Fascin positive tumor foci with more invasive and more prone to lymph node metastasis. 5.Fascin independent prognostic factors affecting laryngeal, can be used as biological indicators judgment laryngeal cancer prognosis. Intervention DARC Fascin expression may block the invasion and metastasis of laryngeal cancer cells, is expected to become a new target for laryngeal cancer.
