Abstract


Through processing cadaver specimens perfused pineal region microscopic anatomy , to further clarify the microscopic structure of the pineal region , especially in the pineal region vein and membrane structures ; while summarizes occipital transtentorial approach to remove pineal region tumor surgery treatment experience , improve the therapeutic effect of the pineal region tumor surgery . This study is divided into two parts : The first part of the pineal region microscopic anatomical study , pineal region venous observed under a surgical microscope magnified 10 times , intravenous clearance , the arachnoid and vein wrapping relations . Measure the pineal region related vein diameter and length ; 21 cases of the second part is a review of neurosurgery of our hospital from September 2006 to September 2008 on the occipital craniotomy transtentorial approach for surgical removal of the pineal region tumor clinical data , intraoperative video data and early postoperative follow-up results were analyzed. 1 great cerebral vein surface thickened arachnoid due to arachnoid double layer surrounding the great cerebral vein and its tributaries trunk . Great cerebral vein , the last paragraph of the bilateral basal vein , bilateral cerebral venous last paragraph as a cover in the pineal region above the outstretched \junction. 2.21 cases of surgical patients with pathologically confirmed 17 cases of germ cell tumors , glioma three cases , pineal cyst . Postoperative hydrocephalus intracranial hypertension ventricular drainage two cases , ventricle peritoneal shunt two cases . Review of 19 cases of postoperative 5d MRI , CT review two cases , total removal of the tumor . All patients discharged recovered well , intracranial hypertension symptoms disappeared , improved hearing and the Masonic movement , but seven cases there are still vertical eye movement disorders , two cases of blurred vision . Conclusion This study confirmed to the great cerebral vein and its the branched trunk surface thickening of arachnoid due to arachnoid double layer surrounding . Occipital transtentorial approach for surgical removal of the pineal region tumor unobscured , exposed to a wide range of protection under direct vision of deep vein timely processing of tumor feeding artery , suitable for the removal of any size and growth direction of the pineal District tumors, especially suitable for large tumors and pediatric patients .
