Objective: To adopt the way the entire complex external fixation with cut reduction and internal fixation of ankle fracture dislocation supination - comparative study of the clinical efficacy of external rotation type Ⅲ, Ⅳ degree a reliable treatment of the fracture type . Methods : September 2007 to December 2008 , our hospital ankle fracture dislocation supination - external rotation type Ⅲ, Ⅳ degree 60 patients met the inclusion and exclusion criteria, with a random number table is divided into two group , 30 cases in each group using manipulative plaster external fixation ( conservative treatment group) , and the other group was treated with open reduction and internal fixation ( surgical group ) , the general data of the two groups of patients , the treatment of data and postoperative functional recovery assessment and comparison . Results : Two groups of patients were followed up for two groups in joint function , pain , X-ray fluoroscopy number of X -ray film , complications , and postoperative ankle function Mazur rated excellent rate differences had no statistical significance ( P gt ; 0.05) but in the number of days of hospitalization , treatment costs , P lt ; 0.01 , there is a very significant difference . Conclusion: manual reduction and plaster external fixation after conservative treatment of ankle fracture and dislocation of the spin - external rotation type III , no significant difference in the clinical efficacy of surgical treatment of degree IV , no significant effect on functional recovery of the choice of two treatments for ankle fractures able to achieve very good therapeutic effect . But conservative treatment safety factor , low-cost economy , better to reduce health care costs of patients , for the purpose of creating more economic benefits to the community .
