Abstract


Background Colorectal anastomotic leakage is the most serious complications of colorectal surgery. With more and more specialized colorectal surgeon, complications after colorectal surgery dwindling, but the incidence of anastomotic leak was still no downward trend. Assessment is still facing a long-term task of the surgeon, to lead to colorectal surgery anastomosis mouth leakage occurred due to different reports of port leakage risk factors lead to colorectal surgery anastomosis over a long period of time, remains controversial. Anastomosis plane of the purpose of study colorectal surgery colorectal anastomotic leakage; prophylactic transverse colon colostomy or not, and the gender of the rectum in the lower segment of anastomotic leakage. Methods A retrospective analysis of General Surgery of the Second Clinical Medical College of Shanxi Medical University from June 1998 to June 2008 478 cases of colorectal surgery patients, rectal surgery patients were divided into three groups according to the different surgical anastomosis plane will bear, consistent plane transverse colon lower segment segment on the sigmoid colon or rectum, rectal anastomosis plane of colorectal surgery for colorectal anastomotic leakage and prophylactic transverse colon colostomy or not using X2 test analysis and gender of the rectum in the lower segment of anastomotic leakage. Results consistent plane respectively, transverse colon, sigmoid colon or rectum paragraph, the following paragraph in the rectum (not prophylactic transverse colon ostomy) the incidence of anastomotic leakage of the three groups were 0%, 2.7%, 8.4%, there was significant difference between the three (P lt; 0.05), transverse colon group sigmoid colon or rectum segment group, the incidence of anastomotic leakage was no statistical difference (P gt; 0.0167), the group of the next segment in the transverse colon group and rectum incidence of anastomotic leak exists a significant difference (P lt; 0.0167), the the segment group sigmoid colon or rectum and rectum under segment group, the incidence of anastomotic leak was no statistical difference (P gt; 0.0167); the rectum under paragraph group prophylactic transverse colon colostomy anastomosis port leakage occurs rate of 3.2%, and did not undergo preventive transverse colon colostomy rectum under paragraph group the incidence of anastomotic leak (8.4%) with no statistical difference (P gt; 0.05); prophylactic transverse colon colostomy rectum in the next paragraph The group of female incidence of anastomotic leak (4.5%) incidence of anastomotic leak men (10.6%) no significant difference (P gt; 0.05). Conclusion colorectal surgery anastomotic leakage occurred plane coincide closely related to the lower anastomosis plane, the higher the incidence of anastomotic leak; surgical prophylactic lower segment transverse colon colostomy and rectum can not effectively reduce anastomotic leakage occurred rate; incidence of anastomotic leakage in the lower segment of the rectum surgery, sex is not the decisive factor.
