Background: severe traumatic brain injury (sTBI) is the focus of the field of neurosurgery and one of the difficulties , the mortality , morbidity are high, a serious threat to human health. Neuronal damage after traumatic brain injury have both primary injury secondary injury , now it has been recognized that the prevention and treatment of secondary brain injury is crucial to improve the prognosis of patients sTBI . Magnesium ions through a variety of mechanisms to reduce secondary injury after sTBI is currently a hot research neuroprotective agents , one study showed that brain injury concentrations of serum TNF-α magnesium sulfate can be applied to suppress . Objective : To observe the serum of patients with severe traumatic brain injury , tumor necrosis factor α (TNF-α) concentration in the dynamic changes and their impact treated with magnesium sulfate , and to explore the exogenous part of the mechanism of magnesium ion therapy . Methods: 60 patients with acute severe traumatic brain injury patients were divided into conventional treatment group and the group treated with magnesium sulfate . All cases were before treatment , the first 1,3,7 days of serum TNF-α levels . Magnesium sulfate treatment group within the first 15 minutes 2.5g intravenous infusion of magnesium sulfate, magnesium sulfate, followed by 10g 24-hour continuous infusion, continuous treatment for 7 days. In addition to application of the conventional treatment group treated with magnesium sulfate, magnesium sulfate outside the other treatment groups are identical, refer to \Monitoring of patients during hospitalization TNF-α levels , record 3 months after injury patients Glasgow Outcome Scale (GOS). Results: Group 1 days elevated serum TNF-α levels , the first seven days a downward trend, the magnesium ion therapy serum TNF-α concentrations were lower than the conventional therapy group (P <0.05). GOS score lower than the conventional treatment group, but not statistically significant. Conclusions: The proinflammatory cytokines and traumatic brain injury , especially severe traumatic brain injury in the early clinical inflammatory pathophysiology closely related. Exogenous magnesium sulfate therapy can increase blood TNF-α decreased neuroprotective effect and play with a trend to improve the prognosis .
