Abstract


Objective: detection of cirrhosis and portal hypertension hypersplenism in patients with liver and spleen tissue iron content in liver tissue transferrin receptor expression of tumor necrosis factor-α protein and gene expression level and blood, to explore different degrees hypersplenism hypersplenism of cirrhosis and portal hypertension hypersplenism iron metabolism in patients with disorders of the mechanism for cirrhosis. METHODS: Experimental groups: healthy control group, mild hypersplenism Group, moderate hypersplenism Group and severe hypersplenism group; each group preoperative and postoperative self-control. RT-PCR and immunohistochemistry was used to detect the expression of the transferrin receptor (TfR) gene and protein levels; atomic absorption spectrophotometer measurements spleen, liver tissue iron content; ELISA assay cirrhosis hypersplenism patients with peripheral venous cirrhosis cytokines tumor necrosis factor-α (TNF-α) expression. Application SPSS 13.0 statistical software of Medicine differences and correlation analysis of gene and protein expression. Results: The liver and spleen tissue iron levels were compared three groups of cirrhotic patients than in the control group, the results were significantly different (P lt; 0.05); mild and moderate hypersplenism group differences in the iron content of the spleen was no significant (P gt; 0.05), may be associated with cases number less related to, among other groups iron content were significantly different (P lt; 0.05); iron content in liver tissue with the degree of hypersplenism increase the iron content increased (P lt; 0.05 ), spleen tissue iron content increased with the iron content in liver tissue increased significantly correlated (correlation coefficient r = 0.741, P lt; 0.01); RT-PCR results showed that the results were statistically significant among the groups TfR mRNA expression levels (P lt; 0.05) of TfR mRNA expression hypersplenism group were higher than the normal control group, between three groups Hypersplenism groups with hypersplenism deepening, TfR mRNA expression in turn increase (P lt; 0.05); hypersplenism liver TfR protein expression organizations immunohistochemistry results were higher than the normal control group; compare the surface density among the three groups of hypersplenism no significant change (P gt; 0.05), while the average gray significantly reduced, the difference was significant (P lt ; 0.05), and integrated optical density was significantly higher, the difference was statistically significant (P lt; 0.05); blood samples of tumor necrosis factor-α (TNF-α) levels were compared between each group differences were significant significant (P lt; 0.05), three sets of samples of cirrhosis and portal hypertension were higher than the normal group, and three group with the hypersplenism of portal hypertension and hypersplenism severity, tumor necrosis factor (TNF-α) the absorbance in turn increase (P lt ; 0.05). Hypersplenism group preoperative and postoperative were statistically significant (P lt; 0.05). Peripheral blood TNF-α expression increased with elevated liver and spleen tissue iron content there is a significant positive correlation (P lt; 0.01). Conclusion: patients with cirrhosis and portal hypertension hypersplenism hypersplenism spleen lead to liver iron overload and liver damage; iron overload caused by elevated expression of TNF-α, causing liver fibrosis and sclerosis; The results of this study support the early removal, severe patients with hypersplenism spleen view.
