Abstract


Objective:Find the appropriate minute ventilation during mechanical ventilation with general anesthesiaMethods:75 patients scheduled for surgery were randomly divided into 5 groups:Group A(Minute Ventilation,MV=60ml/kg),Group B (MV=70ml/kg),Group C(MV=80ml/kg), Group D (MV=90ml/kg) and Group E(MV=100ml/kg).After general anesthesia induction and intubation, adjust tidal volume and respiratory rate to achieve the desired minute ventilation. When mechanical ventilation 5min,30min,60min,90min,120min and before the end of surgery record peak airway pressure (Peak), mean airway pressure (Pmean), plateau pressure (Pplat), HR, MAP, SPO2 and PetCO2, and calculate lung compliance, including:dynamic lung compliance (Cdyn)=tidal volume/(Peak-Peep) and static lung compliance (Cstat)=tidal volume/(Pmean-Peep), arterial blood was collected at the same time to observe the result of arterial blood gas analysis.Result:1. Peak airway pressure (Ppeak), plateau pressure (Pplat), mean pressure (Pmean) changes Ppeak, Pmean, Pplat consistent trend, Each group at all time points there was no significant difference (P> 0.05), Compare Ppeak, Pplat, Pmean in A, B, C, D, E groups which with the increase on tidal volume has significantly increased, there are statistical significance (P<0.05).2. SPO2, PetCO2, the result of arterial blood gas analysis’changes.The SPO2 and PaO2 were no statistically significant difference (P> 0.05) in each group’and groups’. PH in groups increases with minute ventilation, there are statistical significance (P<0.05).Comparie each group’results of PetCO2, PaCO2 at the same time point (T1, T2, T3, T4), the results according to the increase in minute ventilation was significantly lower (P<0.05);PetCO2, PaCO2 within each group when compared between different time points were not significantly different (P>0.05).Conclusions:The appropriate minute ventilation during mechanical ventilation maybe 60ml/kg~80ml/kg (median 70ml/kg)
