Abstract


Objective:To investigate the value of dynamic electrocardiogram (DCG)in diagnosing coronary heart disease(CHD).Methods:There were 120 patients were enrolled who have been examined by DCG before coronary arteriography,and there were 76 cases of the 120 people had been diagnosed hypertension before hospitalization. DCG data of ST-T wave changes, ventricular arrhythmia and atrial arrhythmia were compared with that of coronary arteriography (CAG) in 120 patients using statistic analysis. Results:The sensitivity of DCG in diagnosing CHD is 69.9%, the specificity is 85.2%; 32 patients had single vessel disease,18 of patients positive by DCG(56.3%); 37 patients had two vessel disease,25 of patients positive by DCG(67.6%); 24 patients had three-multivessel damage,22 of patients positive by DCG(95.7%); there was significantly difference(P< 0.05); Positive rate of diagnosing coronary heart disease、single vessel disease and two vessel-multivessel damage of CHD were 72.7%、59.5%and 46.3% in 44 cases of transient antrum pyknosphygmia, respectively. Positive rate in diagnosis of CHD、single vessel disease and two vessel-multivessel damage of CHD were 100%、23.1% and 76.9% in 13 cases of multifocal ventricular premature, respectively; Positive rate in diagnosis of CHD、single vessel disease and two vessel-multivessel damage of CHD were 100%、28.6%and 71.4% in 7 cases of frequent ventricular premature or non-sustained ventricular tachycardia, respectively; There were 46 cases who were examined positively by DCG in 69 patients of CHD among 76 hypertension patients.12 cases were diagnosed positive by DCG in 30 patients of CHD of non-hypertension, the difference was significant(P< 0.05).Conclusion:It has highly sensitivity and specificity in diagnosing CHD by DCG; The positive rate of DCG in diagnosing CHD increased with more vessel lesion of coronary artery; ventricular arrhythmia and atrial arrhythmia in the diagnosis of CHD has more important value; DCG can earlierly detect coronary artery disease for the patients with hypertension.
