Abstract

The purpose of neonatal hypoxic-ischemic encephalopathy (hypoxia ischemic encephalopathy, HIE) can not only cause of perinatal neonatal death, cause one of the main reasons for handicapped children after the neonatal period. A long time, due to the economic situation, the parents agree with the variety of factors such as interference, the neonatal period HIE treatment was generally more complete and timely treatment in the neonatal period is difficult to adhere to the easily overlooked. The topics to be adopted after the neonatal period continued treatment (including drug and non-drug) for neonatal hypoxic-ischemic encephalopathy (HIE) prognosis. Explore the improve HIE prognosis. Method pediatric neonatal ward to meet the diagnostic criteria, and severe in children with neonatal hypoxic-ischemic encephalopathy in the neonatal period the end of treatment, according to the parents' views and the actual situation of the families of children divided into observation group and the control group ( Group C); observation group was divided into two groups: group A and group B. Three groups of children with treatment based on neonatal hypoxic-ischemic encephalopathy treatment plan (draft) to be treated. Comprehensive treatment within 3 days after birth, support therapy (respiratory support, circulatory support and metabolic support) and symptomatic (Zhijing, dehydrating agents and eliminate brainstem symptoms) processing; since four days plus Franc Zellers 5ml / d, adding 30ml liquid infusion, Danshen 1.5ml/kg · d adding 30ml liquid infusion. The moderate HIE course of 10 to 14 days the severe HIE course of 3 to 4 weeks. After birth of all children in 5 to 7 days, 14 days, 28 days line the NBNA 20 neonatal behavioral neurological assessment test. A, B group of children are the same early intervention; interventions: A, B-group children, audio-visual, perception, memory, and motor training; insist on doing the massage exercises, comprehensive treatment of the different-month-old baby gymnastics and physiotherapy. A group of children in the neonatal period were given three courses of drug treatment, drug Franc Zellers 5ml / d intravenous treatment for 10 days; the C routine care group after the neonatal period without medical intervention. The three groups of patients were followed-up in early childhood clinic. 1,6,12 months follow-up of 3 years old. Intellectual development was assessed using infant mental development scale (CDCC), to the mental development index (MDI) and psychomotor development index (PDI) score. Compare 12 months, 3 years old, three groups of children with MDI and PDI. Based on the results of the statistical analysis of the results of 20 neonatal neurobehavioral tests (NBNA) Rating: 5 to 7 days after birth, less than 35 points by 53.95%, 46.05% ≥ 35 points; 12 to 14 days after birth, less than 35 points by 21.05%, 78.95% ≥ 35 points; 24 to 28 days less than 35 points after birth by 14.47%, 85.53% ≥ 35 points; With the age increasing, NBNA ratings gradually increased. All patients completed the follow-up of 12 months, December CDCC test: A group of MDI to 92.027 ± 8.48, PDI 93.216 ± 9.096 87.844 ± 7.89; B group MDI, PDI 88.500 ± 8.474; the C groups MDI is 83.821 ± 8.693, PDI to 84.231 ± 9.068. The observation group A, B two groups were higher than group C after the neonatal period for medical intervention, combined with three courses of drug therapy in children with group A is better than group B only training intervention group. Moving and parents change in address, 3 years old observation group (all group B), the control group, seven people lost contact, 3-year-old CDCC test: A group of MDI 97.59 ± 8.441, PDI to 98.16 ± 7.544; group B MDI is 93.55 ± 7.124 PDI to 88.500 ± 8.474 88.94 ± 7.586; C group MDI, PDI was 89.75 ± 7.304. Group C without medical intervention in the observation group A, B groups were higher than the neonatal period, with 12 months in group A and group B, PDI was no significant difference in the 3 years old, MDI3 age group A still higher than the B group. Conclusions (1) disease early, NBNA score higher abnormal rate, gradually raise the the NBNA score after 2 weeks of treatment, two weeks after NBNA abnormal baby, NBNA ratings also been restored, after further medical intervention, such as 24 the -28 days NBNA score is still less than 35 points, the prognosis is poor. (2), severe neonatal hypoxic-ischemic encephalopathy should adhere to a sustained and comprehensive treatment in the neonatal period, can improve children's intelligence and athletic ability levels to improve HIE prognosis, drug therapy combined with early intervention treatment than simply The effect of early intervention, especially for mental development, psychomotor developmental little difference between the two.
