Abstract


The reform and improvement of basic medicine and care system in cities and towns is the inevitable requirement of rapidly development of our national economy and the inevitable trend of further deepening and development of our national medicine and care reform; besides ,It is even the important measure of our Party and Government to put "People oriented " scientific concept of development into effcet, care about people’s livelihood, improve people’s livelihood, and pay attention to social fairness and justice. Researching on the current dilemma of our national urban basic medical insurance, analyzing the root , and exploring the new way to keep the medical insurance development healthy and order are the important and urgent subjects the reality put forword.Nowadays, There are many difficulties in the development of our national urban basic medical insurance , the main reasons are as follow: the expenses of the basic medicine are growing too fast, and the fairness of urban residents to seek medical treatment are decling; the basic medical resources are lacking and configuration are unreasonable; the public welfare of the public hospitals are decling and its profit-enhancement are increasing ; the basic medical insurance level is low, and the system is unperfect; the disorder of the medical production and circulation leads to virtual-high drug prices. The reason is caused by the interaction of a variety of factors. There are three reasons : firstly, the governmental functions are absence in policy builting, economic investment, medical supervision and so on. Secondly, there are many negative effects existed in marketization reform of urban basic medicine. Thirdly, the current management system and operational mechanism of urban basic medicine has defects.Based on the problems that exist in the development of our national urban medical insurance, Zichang County responding to the state call , according to the actual situation of the country ,actively promote the reform of urban basic medical insurance, and Zichang has explored a new path of its urban medical system reform: In the area of medical service system, it focus on the foundation construction of medical institutions and the construction of the talent team; In the area of the reform of medicine-on-credit system, it established a system that concentrated procurement and unified distribution of the drug; In the area of perfecting the medical protection system, it expanded the coverage of basic medical protection, improved the protecting level, and reformed the personnel system; In the area of strengthening the protection way of medical system reform, it strengthened the organization’s leadership, and established a strict and effective mechanism of medical supervision. The effectiveness of Zichang County’s reform of urban medical system is worthy of recognition: Establishing the "price hospital", which truly benefits patients; paying attention to perfecting the system of medical services, which improved the capacity of medical services; Establishing a system of basic drug prices in the county, which prevented the unhealthy trends in the field of medicine-on-credit; perfecting the medical protection system, which raised the level of medical protection; Strengthening the dominant position of the government, which strengthened the public welfare of public hospitals.As a typical example of the reform of our national urban basic medicine system, Zichang County’s successful experience has certain popularity and feasibility. Referencing Zichang County’s successful experience of medicine system reform, if we further promote and deepen the reform of our national urban basic medicine system, we must do the following six points: Adhering to put "people-oriented" as the purpose of medical system reform, and always starts from patients’vital interests, effectively services for the majority of patients; Adhering to put government-led as the key of urban medical system reform to guarantee the direction of urban medical system reform; Adhering to put system construction as the fundament of urban medical system reform, and establish a health insurance system which is suit for our country ; Adhering to create the "price hospital" as a platform, and largely solutes patients’problems that it is difficult and expensive to seeing a doctor ;Adhering to put the supervision of medicine market as the hand of urban medical system reform, and realizing the standardization and rationalization of medicine market; Adhering to medical talents flowing and perfect medical service system vigorously, and then improve the ability of medical service.
