Abstract

Objective: obsessive-compulsive disorder (OCD) and bipolar disorder (BD) are common with obvious genetic predisposition and lead to significant cognitive impairment, mental disorders, both often mutually comorbidity. This study intends to explore the cognitive function of OCD and associated with BD comorbidity of OCD patients with normal cognitive function differences; explore OCD associated with BD comorbidity with OCD 5 - serotonin transporter promoter region (5-HTTLPR), children catechol-O-methyltransferase (COMT) gene polymorphism and its differences with the normal. Methods: The patients with OCD alone group (referred to as the OCD group, n = 81): in line with DSM-IV forced disorder diagnostic criteria patients have finished composing the questionnaire (including demographic data and epidemiological data), HAMD, HAMA, The WCST, BRMS, Y-BOCS and obsessive-compulsive symptoms list; connection test A, B, and Wechsler Intelligence Test blocks puzzle subtests assess the patient's cognitive function. Associated with BD comorbid OCD group (referred to as comorbidity group, n = 74): patients meet the diagnostic criteria of the DSM-IV obsessive-compulsive disorder and bipolar disorder, also finished composing the questionnaire (including demographic data and epidemiological data etc.), of HAMD, HAMA, BRMS, Y-BOCS and obsessive-compulsive symptoms list; using WCST connection test A, B and the Wechsler intelligence tests blocks puzzle subtests assess their cognitive function. Normal control group (referred to as the normal group, n = 112): Select age, gender and education level and patient group basic match, no mental illness Past medical history and positive family history, there is no history of psychoactive substance dependence healthy people, SCL- 90 screening no significant mood disorder or obsessive-compulsive symptoms and assessed the Y-BOCS (Chinese Edition) Score lt; 6 points were. Serious physical illness or physical disability, color blindness and poor compliance by excluded by physical examination. Into the group of normal controls must record the gender, age, occupation, education, years of general information; using the WCST, the connection test A, B, and Wechsler Intelligence Test blocks puzzle subtests assessing cognitive function. Above all the subjects were right-handed Chinese Han people, the majority of patients and normal subjects enrolled taken 5ml venous blood used to extract DNA for molecular genetic analysis purposes. Complete the sample into the group after the the OCD group comorbid group part of the clinical features (including the course of the disease, family history, age of onset, comorbidity and substance abuse, and suicide) associated with self-injury compare; OCD group, comorbidity group and normal The group cognitive function of the various test results for pairwise comparisons, understanding the cognitive function of the difference between the three groups. Finally, the three groups of subjects DNA by polymerase chain reaction (PCR) to detect the 5-HTTLPR and COMT gene polymorphisms and their genotype and allele frequencies of 5-HTTLPR and COMT gene analysis. Results: ① comparison OCD group and common part of the clinical features of disease group (including the course of the disease, family history, age of onset, comorbid substance abuse and suicidal behavior associated with self-injury), the course of disease comorbidity group than OCD patients longer. Test scores compared with the normal group, the paradigm shift in the OCD group cognitive function of time, A pen number, B pen number significantly inferior to the normal group, while no significant difference between the two groups in other indicators. Normal group and common disease groups of subjects in a conversion time of thinking, A pen number, A connection total time, B connections total time, number of errors B, B pen number, WCST number of categories, WCST perseverative errors, WCST total The differences in the test scores of the number of errors and the WISC building blocks Score reach a statistically significant degree of comorbidity group is inferior to the normal group. The of comorbid OCD group group A number of errors, number of errors B test differences in the performance of the WCST non-perseverative errors did not reach a statistically significant degree, other indicators have significant differences, comorbidity group is inferior to the OCD group. ③ three sets of 5-HTTLPR genotypes were in Hardy-Weinberg equilibrium law (χ2 = 2.723, df = 1, p = 0.098; χ2 = 2.668, df = 1, p = 0.102; χ2 = 0.001, df = 1, p = 0.980); three groups COMT genotype also consistent with the Hardy-Weinberg equilibrium law (χ 2 = 0.001, df = 1, p = 0.971; χ 2 = 0.207, df = 1, p = 0.649; χ 2 = 2.118, df = 1, p = 0.146). ④ 5-TTLPR and COMT gene genotype and allele frequencies in the normal group, no significant difference between the OCD group and associated with BD comorbid OCD group. (5) The results show the ,5-HTTLPR and COMT genotype and allele frequencies in the normal group, OCD group and accompanied BD comorbid OCD group also no significant difference in gender distribution. Conclusion: (1) with BD comorbid OCD patients compared with patients with pure OCD course of the disease is more persistent; (2) OCD group comorbid patients have defects of cognitive function, the comorbidity group cognitive function compared to the OCD group recognized worse cognitive function, indicating that BD and OCD comorbidity on cognitive function in patients with the superposition of the nature of the damage; (3) 5-HTTLPR and COMT genotype and allele frequency distribution in the OCD group associated with BD comorbid OCD group no significant difference; (4) 5-HTTLPR and COMT genotype and allele frequency distribution with OCD and comorbid BD comorbid OCD patients gender unrelated.
