Abstract


Objective: To study granulosa cell mitochondrial activity of egg, embryo development and in vitro fertilization - embryo transfer outcomes; study the correlation of the level of L-carnitine and granulosa cell mitochondrial activity; investigate plasma L-carnitine levels in follicular fluid of in vitro fertilization - embryo transplant outcomes. Methods: From December 2008 to March 2009 in our hospital reproductive center due to tubal infertility in vitro fertilization - embryo transfer (IVF-ET), the normal menstrual cycle, 63 patients (63 cycles), excluding endometriosis, endocrine disease, and patients with male infertility factors. Application of gonadotropin-releasing hormone agonist long protocol ovarian hyperstimulation, egg retrieval date follicular fluid collected plasma Eye view without bloodshed, the application of pre-column derivatization - high performance liquid chromatography (HPLC) determination of free L-carnitine levels. The ovum while collecting patients with follicular fluid density gradient centrifugation granulosa cells, flow cytometry assay mitochondrial activity of granule cells. Study treatment outcomes into the pregnancy group and the non-pregnant group were compared plasma L-carnitine levels in follicular fluid and follicular fluid, granulosa cell mitochondrial activity, analysis of mitochondrial activity of granule cells in follicular fluid and plasma, follicular fluid L-carnitine alkali level of correlation analysis of the three age, number of oocytes, fertilization rate, cleavage rate, and excellent embryo rate correlation. Results: 1 total clinical pregnancy rate of 38.98%, and the average age was 32.24 ± 4.14, a minimum of 24 years old, 44 years old, and the number of oocytes average of 14, minimum 1, maximum 32; excellent rate of embryos in bits 50%; mitochondrial activity average 85.04 ± 10.25%, minimum 55.54%, maximum 98.06%; plasma free L-carnitine levels average 34.23 ± 7.74μmol / L minimum 19.95μmol / L, maximum 52.93 μmol / L; follicular fluid of free L-carnitine levels an average of 24.34 ± 7.08μmol / L, the minimum 8.25μmol / L, maximum 39.89μmol / L,. Pregnant and non-pregnant group compared the pregnancy group granulosa cells mitochondrial activity higher than the non-pregnant group (89.10 ± 7.24% vs 83.22 ± 11.05%, P lt; 0.05), pregnancy group have lower follicular fluid L-carnitine level (21.33 ± 5.83μmol / L vs 26.07 ± 5.46μmol / L, P lt; 0.05) and lower plasma level of L-carnitine (30.82 ± 7.10μmol / L vs 35.69 ± 7.28μmol / L, P lt; 0.05) . 3 granulosa cell mitochondrial activity with age was a significant negative correlation (r = -0.449, P = 0.001), and the number of oocytes excellent embryo rate normal fertilization rate into the positive correlation (P lt; 0.05). Plasma L-carnitine levels with age was a significant positive correlation (r = 0.467, P = 0.002), and granulosa cell mitochondria activity negatively correlated ((r = -0.447, P = 0.012)), and was the number of oocytes negative related (r = -0.432, P = 0.005), and no significant correlation with the fertilization rate, cleavage rate, excellent embryo rate the rest of the relevant parameters of the IVF-ET. L-carnitine levels of follicular fluid with elevated plasma level of L-carnitine, but there was no significant difference (P gt; 0.05), follicular fluid concentration of L-carnitine and normal fertilization rates were negatively correlated (r = -0.284 , P = 0.042), and age, were the number of oocytes retrieved, fertilization rate excellent rate of embryos remaining IVF-ET parameters without significantly related to sex (P gt; 0.05). Conclusion: 1. Reduction of mitochondrial activity in granulosa cells may affect egg development. 2 granulosa cell mitochondrial activity and embryo quality have a close relationship, may be used as indicators to predict embryo quality. Reduction of mitochondrial activity in granulosa cells may be one of the reasons for poor IVF-ET outcomes of older women. Plasma L-carnitine levels increase with age and elevated. 5 plasma and follicular fluid levels of L-carnitine concentration may be increased oxidative damage in the body feedback.
