Value of ABCD~2 Score and Diffusion-weighted Imaging in the Prediction of Stroke Risk after Transient Ischemic Attack

PurposeAssessment ABCD2 score and diffusion-weighted imaging(DWI) in short-term predictive value from transient ischemic attack(TIA) to cerebral infarction.MethodA retrospective analysis was performed on 67 patients with TIA of department of neurology of Shengjing hospital affiliated of china medical university during Oct.2008 to Oct.2009. According to the ABCD2 score of patients, We divide the 67 patients into three group-risk stratification. View the actual stroke rate.A logistic regression model was developed using the actual stroke the response criterion and graded ABCD2 score (low, moderate and high risk) and DWI findings as covariates.To investigate the value of the graded ABCD2 score and DWI in forecasting the risk of stoke after TIA.ResultsSubsequent stroke occurred in 13 cases (19.4%). By ABCD2 score, showing that low-risk group of 28 cases, one case developed a subsequent stroke (3.6%). In the moderate-risk group,10 cases (29.4%) of 34 cases stroke. High-risk group of 5 cases, 2 cases (40%) occurred in stroke 19 cases(28.3%) of the 67 TIA patients had DWI abnormalities. Graded ABCD2 score and DWI were each independent predictors of stroke risk after TIA. The area under the receiver operating characteristic curve(AUC) for graded ABCD2 was 0.724 [95%CI 0.587 to 0.862, P<0.05]. The AUC for the model that combined graded ABCD2 and DWI was 0.815 [95% CI 0.710 to 0.919, P<0.05].ConclusionsThe ABCD2 score is simple and reliable to predict subsequent stroke risk,can serve as patients with TIA to assist in early implementation of therapeutic measures. Combining graded ABCD2 and DWI abnormalities can increase the accuracy of predictions for early risk of stroke after TIA,and the value of the combination model need to expand the sample to investigate.
