IntroductionNeonatal respiratory failure is common in NICU as a serious clinical problem which connected with grow age, ventilation therapies for neonatal respiratory failure have been introduced as the development of neonatory. This study was to investigate indications、complications、outcomes and the factors influencing neonatal mechanical ventilation.MethodsRetrospective observational study conducted on 206 consecutive newborns with neonatal respiratory failure, who required mechanical ventilation admited in the NICU of our hospital from July,2005 to July,2007. We recorded the clinical data and analized them with statistical methods.Results1. The commonest indication for neonatal mechanical ventilation was respiratory distress syndrome(RDS) (29.6%), followed by pneumonia (18.0%), inturaterine pneumonia (14.1%), sepsis (10.7%), meconium aspiration syndrome (MAS) (6.8%), severe asphyxia (6.3%). Overall respiratory diseases accounted for 72.3%. Indications were different between preterm and term infants.2. Most patients of our analysis appeared metabolic acidosis in the arterial blood gas analysis. The time of oxgyen inhalation was increased by the duration of mechanical ventilaition, and the average maximum peak inspiratory pressure and maximum fraction of oxygen concentration were similar (P<0.05) between preterm infants and term infants.3. The incidence of brain injury in preterm infants(49.2%) was significantly higher than that of term infants(33.8%). Of all the infants, the mortality rate was 8.25%,7 died from infection,2 from persistent pulmonary hypertension,2 from pulmonary hemorrhage,2 from pneumothorax, and 2 from multiple organ failure as well as DIC and 2 from severe asphyxia.ConclusionRDS(preterm infants) and pneumonia(term infants) were the commonest indications of infants with mechnical ventilation in our analysis. Ventilated premature infants were more vulnerable to brain damage. Most infants died from infection. It was most important to strengthen antenatal care, prevent preterm birth and infection to reduce the incidence and mortality of neonatal respiratory failure.
