Purpose : through the analysis and comparison of the clinical data of 66 cases of patients with common bile duct stones , evaluation laparotomy and endoscopic treatment of common bile duct stones respective advantages and disadvantages . Methods: choledocholithiasis patients from January 2008 to July 2008 , the First Affiliated Hospital of Dalian Medical University, admitted a total of 66 cases , divided into treatment the laparotomy group therapy and endoscopic treatment group . Compare two sets of data in the clinical features , the success rate of surgery , a stone clearance rate , a common complication incidence , for the first time the eating time whether there was significant difference literature analysis . Results : laparotomy group of 32 patients , 18 men and 14 women , with an average of 68.4 ± 14.2 years , the size of the common bile duct stones 1.35 ± 0.38cm, the CBD diameter 1.33 ± 0.42cm , common bile duct single stones in 14 cases of multiple stones in 18 the cases ; endoscopic group of 34 patients , male 19 cases, female 15 cases , an average of 63.9 ± 9.2 years, the size of the common bile duct stones 1.27 ± 0.24cm, common bile duct diameter 1.24 ± 0.38cm , the common bile single stones in 17 cases , multiple stones 15 cases , clinical data comparing the two groups showed no significant difference ( P > 0.05 ) . The laparotomy group has complications 17 patients (51.3%) ; endoscopic groups complications of 24 patients (75.0%) , and complications in two groups were significantly different ( P <0.05 ) . Laparotomy group success rate of stone and stone clearance rates were 100% , 84.4% : the endoscopy group of stone success rate and a stone clearance rates were 94.1% , 81.3 % , and the two groups this difference was not statistically significant ( P > 0.05 ) . Recent 10 cases of patients with complications after open surgery group , 1 patient died due to complications and ultimately , accounting for 31.3% ; term complications of endoscopic postoperative patients six cases , with no deaths , accounting for 15.6% of the two there was a significant statistical difference ( P <0.01 ) . Laparotomy treatment group feeding time for the first time after endoscopic treatment group were 47 ± 9 hours , 24 ± 6 hours , significant difference ( P <0.05 ) between the two . Conclusion: The incidence of common bile duct stones men and women no significant difference in the incidence increased with increasing age ; laparotomy and endoscopic treatment of common bile duct stones efficacy equalization , but the endoscopic treatment of trauma , fewer complications , for the combined medical illness and the number of treated patients , and more secure. However , for complicated cases of large stones and biliary tract , abdominal treatment should be priority choice.
