Abstract


Background: pleomorphic adenoma is the most common type of salivary gland tumors, recurrence after surgical resection is considered incomplete tumor capsule, caused by the growth of multi-center, surgical options and improper operation surgery 1. But even if taken to save the facial nerve in the parotid gland and tumor resection, still nearly 3% recurrence rate, and recurrence parotid pleomorphic adenoma malignant transformation. So some people think that the tumor itself biological activity is a factor not to be ignored. Ki-67 is a nuclear proliferation-associated antigen, can fully reflect the proliferative activity of the cells, the most studied in recent years, the most promising cell proliferation marker one. Cyclin-dependent kinase 4 inhibitor gene (p16INK4/CDKN2, referred to as p16) is a novel tumor suppressor gene, p16 gene encodes a protein that specifically inhibit CDK4 kinase activity affect the regulation of the cell cycle. Frequent inactivation of the gene in a variety of tumor types, and in recent years, tumor molecular biology research focus. By observing contrast, Ki-67, p16 parotid pleomorphic adenoma recurrence after surgery first (before relapse), second (recurrence) in the surgical specimens without recurrence after normal parotid tissue expression, to explore its relationship with tumor recurrence. Objective: To investigate the Ki-67, the correlation between p16 and the parotid pleomorphic adenoma recurrence biological behavior, in order to provide a reference for the prognostic assessment of parotid pleomorphic adenoma. The standards require surgery: follow-up of 1416 cases of pleomorphic adenoma cases, 17 cases of recurrence, 11 patients with relapsed after paraffin-embedded specimens preserved, choose more than 10 years (mean 12.2 years) without recurrence of pleomorphic adenoma 20 cases and 20 cases of normal parotid gland. Envision immunohistochemistry determined groups of Ki-67 positive expression index (LI) and p16-positive rate, calculation of p16 was strongly positive constituent ratio among groups. Results: (1) pleomorphic adenoma postoperative recurrence rate was 1.20%; (2) Ki-67 LI recurrent parotid pleomorphic adenoma recurrence (2.68 ± 0.50%), the latter group (3.00 ± 1.20%) difference between statistical significance (P gt; 0.05), both compare the differences were statistically significant (P lt; 0.05), respectively, with no recurrence group; recurrence ago, after the group without recurrence normal parotid gland (0.98 ± 0.32%) compare the differences were statistically significant (P lt; 0.05). (3) normal parotid gland p16 positive expression rate (95.00%) was higher than the other three groups: before relapse group 45.45% 36.36% after relapse group without recurrence of 60.00%, and the difference had statistical significance (P lt; 0.05); among the three groups after the pairwise comparisons not statistically significant (P gt; 0.05); of p16 positive strong positive constitute than recurrence former group (0%) and recurrence group (25.0%), no recurrence group (66.7% ) and recurrence after group without recurrence group and the normal the parotid group (89.5%) compared to the difference was not statistically significant (P gt; 0.05). Without recurrence group and recurrence of the former group, before relapse after group were compared with normal parotid group differences were statistically significant (P lt; 0.05). Conclusion: From the study of the experimental sample, pleomorphic adenoma recurrence proliferative activity its own strong relevant; 2.Ki-67 expression index pleomorphic adenoma recurrence likely should be strengthened after follow-up; P16 gene with pleomorphic adenoma occurrence and recurrence, but not the only factor.
