Expression of FOXP3 mRNA in Peripheral Blood Monocytes of Kidney Transplantation Patients with Acute Rejection or Chronic Allograft Nephropathy

Objective: To investigate the relationship between the level of Foxp3 mRNA in the peripheral blood monocytes with acute rejection or chronic allograft nephropathy after kidney allotransplantation, through which to look after a noninvasive biomarker of diagnosing acute rejection and chronic allograft nephropathy. Also we want to provide new theoretical basement for the induction and maintenance of transplantation immune tolerance through this study.Methods: Real-time quantitative polymerase chain reaction (real time-PCR) was used to examine Foxp3 mRNA expression in the PBMCs from 16 cases with acute rejection、8 cases with chronic allograft nephropathy、8 cases at stable stage after kidney transplantation、8 cases of end-stage renal disease and 8 normal controls.Results: 1. The level of Foxp3 mRNA in the PBMCs was significantly lower in patients with acute rejection than that in chronic allograft nephropathy patients.(P < 0.01);2. Foxp3 mRNA expression was significantly lower in patients with chronic allograft nephropathy than that in normal control group、end-stage renal disease group and stable stage group.(P < 0.01);3. While the level of Foxp3 mRNA in the PBMCs among the normal control group、end-stage renal disease group、stable stage group were not significantly different. (P > 0.05).Conclusion: 1. The level of FOXP3 mRNA expressed in peripheral blood monocytes of renal transplantation patients with acute rejection and chronic allograft nephropathy were reduced. It can reflect the status of renal allograft.2. The difference of FOXP3 mRNA expressed in peripheral blood monocytes between acute rejection and chronic allograft nephropathy is obviously , so the level of FOXP3 mRNA expressed in peripheral blood monocytes of renal transplantation patients maybe a noninvasive biomarker for diagnosing acute rejection and chronic allograft nephropathy.3. The reduction of FOXP3 mRNA expressed in peripheral blood monocytes might result in acute rejection and chronic allograft nephropathy post-transplantation, which suggests that FOXP3-targeted therapy maybe a new effective method for inducing and keeping transplantation immune tolerance.
