Abstract


Femoral neck is one of the common orthopedic clinical fracture, occurs mainly in the elderly over the age of 60. With the growth of the average life expectancy, and its incidence is increasing year by year. It has some distinctive features: First, the high average age of the patients, the mortality rate than common fracture; fracture blood supply is easy to destroy, not only affect fracture healing and prone to avascular necrosis and collapse, was 20 % ~ 30%; Third, because of the characteristics on the functional anatomy of the fracture site often withstand large shear stress affect stability fractures and fractures, thus affecting the internal fixation effect internal fixation Lvjing to reform nonunion rate was still high, about 10% to 20% fracture. Elderly femoral neck fracture treatment aims to reduce complications, reduce mortality and improve the quality of life for rehabilitation level and older. Treatment of fractures, the prevention and treatment of osteoporosis in systemic complications three can be neglected, femoral neck fracture elderly patients can not walk, weight, loss of normal life activity, prolonged bed rest can cause pneumonia, bedsores, urinary system infection and other complications, is the main reason for the femoral neck fracture mortality increase. At home and abroad in the treatment of a large number of studies, femoral neck fractures have significantly improved, but the femoral head necrosis yet to make significant progress, now in recent years, some studies are summarized below. This thesis is divided into three parts. The first section describes the anatomical characteristics of the femoral neck, hip biomechanics. Because of the particularity of the anatomical relationship of the femoral neck fracture after femoral head of the local blood supply is compromised, thus ischemic necrosis and nonunion major problems still need to be addressed in the femoral neck fracture. Ischemic necrosis and nonunion occurred with the initial degree of trauma, the extent of the damage of the vascular supply, and the degree of reduction of the fracture was related. To promote the healing of femoral neck fracture, reduce the incidence of femoral head necrosis and nonunion, should consider the following three aspects: (1) local blood supply rehabilitation; (2) the accuracy of the reset; (3) strong fixed. The second part describes the study of Chinese and Western medicine for femoral neck fracture risk factors and pathogenesis. Western medicine for femoral neck fracture risk factors as well as traditional Chinese medicine for the discussion of the etiology and pathogenesis of fractures. It also highlights the pathogenic characteristics of the fracture of the femoral neck. The third section details the progress in the treatment of femoral neck fractures. Non-surgical treatments, including manipulation, traction, fixed internal fixation bloody shipped muscle pedicle muscle flap and vascular bundle graft arthroplasty surgical treatment and fracture the prognosis complications and functional training methods .
