Abstract


Objective To investigate the impact of preoperative obstructive jaundice rats after hepatectomy liver regeneration to provide a theoretical basis for clinical application. Establish the obstructive jaundice SD rat model, the determination of the obstruction is not relative level of jaundice, and grouped according to the the jaundice different level: 1.SO-Hx group: normal rat hepatic resection group, 2.OJ1-Hx groups: obstructive jaundice one day hepatectomy group, 3.OJ3-Hx group: three days of obstructive jaundice liver resection group, 4.OJ7-Hx group: 7 days obstructive jaundice liver resection group; remnant liver weight observed after hepatectomy animal mortality / weight ratio, Biochemical detection of postoperative liver function, immune histochemical detection of postoperative liver tissue PCNA labeling index, RT-PCR was used to detect liver tissue HGF mRNA expression. Results induced by bile duct ligation the biliary obstruction SD serum total bilirubin was progressively increased, the slow decline obstruction 168h (7d), reached the peak (about 85 times). Biliary obstruction underwent liver resection increased mortality the OJ1-Hx group, OJ3-Hx group and OJ7-Hx group mortality rates were 6.67%, 26.67% and 46.67%, significantly higher than that in the control group (0%) ; the OJ3-Hx groups and the OJ7-Hx group liver resection 168h (7d) of the right lobe of the liver weight / body weight ratio was significantly lower than the SO-Hx group, the difference was significant (p lt; 0.05) OJ1-Hx group close to the SO-Hx group, the difference was not significant sex (p gt; 0.05);-Hx group OJ3 OJ7-Hx group liver resection liver function recovery was slow, 0-168h after SO-Hx group, the difference significant (p lt; 0.05); liver function recovery OJ1-Hx group relatively rapid postoperative 168h (7d) compared to the SO-Hx group, the difference is no longer significant sex (p gt; 0.05); OJ3- Hx group and OJ7-Hx groups of PCNA labeling index and expression of HGFmRNA recession peak level after the peak expression are weak, compared with the SO-Hx group, the differences were statistically significant (P lt; 0.05); PCNA labeling index OJ1-Hx group and HGF mRNA expression close to the SO-Hx group after hepatectomy 168h (7d), there was no significant difference sex (p gt; 0.05). Conclusion 1. The obstructive jaundice animals induced by bile duct ligation model, capable of simulating hilar cholangiocarcinoma with obstructive jaundice pathophysiologic characteristics, repeatability, and stability is good; 2. Biliary obstruction caused by high bilirubin hyperlipidemia significantly inhibited liver regeneration after hepatectomy, the degree of inhibition and the time of obstruction and jaundice level related to the longer time of obstruction, the higher the level of jaundice, the inhibitory effect of the stronger; 3. preoperative obstruction short time in bilirubin a low level, may be relatively safe hepatic resection.
