Abstract

Objective: To investigate the detection of ischemia modified albumin (IMA) on the value of early diagnosis of acute coronary syndrome (ACS). Methods: create a White Rabbit myocardial ischemia-reperfusion animal model, ischemia-modified proteins in the process of myocardial ischemia trend observed in animal experiments. Analysis of 277 cases of chest pain to the Hunan Provincial People's Hospital and the Central South University Xiangya Second Hospital emergency department patients with suspected ACS, its treatment within three hours of venipuncture, after centrifugation serum were detected ischemia modified albumin ( IMA), creatine kinase (CK), heart-type creatine kinase isoenzyme (CK-MB), troponin Ⅰ (cTnI) and myoglobin (MYO), to observe the content changes and for comparison, observation and analysis 277 patients, 226 patients diagnosed by coronary angiography in the examination and treatment after admission for ACS patients, another 51 patients diagnosed with non-ACS patients, and other selected 202 healthy people in the same period of examination to detect serum ischemia The modified albumin as a control group. Ischemia modified albumin detect albumin cobalt binding test (ACB), the measurement results ACB units report. ACB units decrease reflects IMA elevated levels. Results: animal experiments can be observed that the dynamic curve of myocardial ischemia occurs when the White Rabbit blood circulation ischemia modified albumin reflected by ACB units. Myocardial ischemia occurs when the White Rabbit blood circulation in ischemia modified albumin increased the lead the ACB value continued to decline, in the case of new myocardial ischemia occurred with blood reperfusion continuing, ACB values ??rebound after a period of time, This experimental study changes in line with the foreign literature. In the analysis of 277 cases of clinical patients with chest pain performance ischemia modified albumin observed, through a relative operating characteristic curve (ROC curve) obtained when the ACB of the cut off value 67.6U/mL albumin cobalt binding test detection method (ACB assay) detection of IMA sensitivity and specificity of the largest, is the diagnosis of the most efficient, error least optimal threshold, the area under the ROC curve at this time was 0.882, sensitivity 90.7%, specificity of 68.6%. Analysis of the healthy control group, the the ACB value of ACS diagnosed group and non-ACS patient group serum detected statistically significant differences in H = 340.089, P <0.01. ACS patients in 226 cases diagnosed, ACB units reduce the reach patients more than the diagnostic threshold of 205 cases, the positive rate was 90.7%, CK, CK-MB, MYO and cTnI positive rate of 19.5%, 32.7% , 16.8% and 46.5%, respectively. Five myocardial ischemia biochemical indicators in the onset of chest pain patients with ACS within three hours of the change in both statistically significant differences in X to 2 = 318.901, P <0.01. When ACB separately or simultaneously in combination with the other four myocardial ischemia markers, the detection indicate early myocardial ischemia, the positive rate increased to 90%. Conclusion: Ischemia modified albumin can produce rapid onset of myocardial ischemia and rapidly released into the blood circulation drill available albumin binding assay, an early indication of the role of acute coronary syndrome is greater than the more commonly used myocardial missing Blood biochemical markers CK, CK-MB, MYO and cTnI, is a sensitive early diagnosis of acute coronary syndrome biochemical markers.
