Abstract


Objective: The purpose of this study is of major vascular and neural structures in the nasal endoscopic positioning pterygopalatine fossa , pterygopalatine fossa region for endoscopic sinus surgical approach to provide anatomical basis . Materials and Methods: intranasal endoscopic six fresh cadaveric head 12 flanking fossa anatomy, anatomy staining silicone intravascular infusion . Completed intranasal endoscopic maxillary sinus posterior wall of the approach , the expansion of the posterior wall of the maxillary sinus approach and the lateral nasal wall dissection approach the study of comparative anatomy and pterygopalatine fossa blood vessels and nerves structure anatomical positioning . Results: sieve crest and sphenopalatine foramen , the infraorbital canal carina as well as the internal maxillary artery pressure trace three important signs of major blood vessels in the nasal endoscopic positioning pterygopalatine fossa neural structures . The nasal endoscopic posterior wall of the maxillary sinus approach can reveal the structure of the upper part of the pterygopalatine fossa , the expansion of the posterior wall of the maxillary sinus approach and the lateral nasal wall dissection approach might be revealed pterygopalatine fossa region structure and temporal fossa medial zone structure, the former for the exposure of the inner region has an advantage for the exposure of the outer region , the latter have the advantage . The lateral pterygoid muscle plane is the nasal endoscopic transnasal - the outer boundary of the maxillary sinus approach . Conclusion: The anatomical studies have shown that these endoscopic signs the pterygopalatine fossa within the positioning of major vascular and neural structures is very important , enabling the surgeon to enter the pterygopalatine fossa region of the underlying blood vessels and nerves more precise positioning and reduce complications . Preoperative extent of disease according to the pterygopalatine fossa , the inferior turbinate and nasolacrimal duct are violated , the extent of the lesion within the maxillary sinus and the type of surgery four main factors , select the most reasonable intranasal endoscopic nose - maxillary sinus surgery approach .
