Abstract


Objective: through experimental research and clinical studies, the concept 察阿瓦斯汀 (Avastin) security applications in the eye. : To give the rabbit vitreous cavity injection different doses (1.25mg/0.05ml 2.5mg/0.1ml 5mg/0.2ml) of Avastin and anterior chamber injection 1.25mgAvastin 7 days before the injection and after injection were performed corneal endothelial mirror , OCT and ERG examination, and 7 days after injection, removal of the rabbit eye by light microscopy and transmission electron microscope examination of the corneal endothelial cell density, the area of ??the coefficient of variation, OCT measured retinal the thickness, ERGb wave amplitude data for statistical analysis. Select 29 cases (30) in patients with diabetic retinopathy, clinical given intravitreal injection Avastinl.25mg/0.05ml. Before injection, after injection of 3 days, 1 week, 1 month, in March, its microscopic examination of the corneal endothelium to the observation area of ??the selected cornea corneal endothelial cell density, the area of ??the coefficient of variation, an area of ??the standard deviation of the maximum area, minimum changes in the area, and other indicators; injection before and after injection January line ERG, OCT and fundus fluorescein angiography, to changes observed ERGb wave amplitude and the peak time and macular edema. ERG data and OCT data on the corneal endothelium indicators data for statistical analysis. : Rabbit vitreous cavity and anterior chamber injection intravitreal injection 5mg group and anterior chamber injection group corneal endothelial cell density was significantly reduced, significantly increased the coefficient of variation of cell area; 1.25mg and 2.5mg group had no significant change. The decrease in anterior chamber injection corneal endothelial cell density, area coefficient of variation increases the difference was statistically significant (p lt; 0.05) compared with the other groups. Rabbit vitreous cavity after injection 5mg group the experimental eyes OCT measured retinal thickness was significantly thicker, the difference was statistically significant (p lt; 0.05); no significant changes in the other groups, the difference was not statistically significance (p gt; 0.05 ). Rabbit vitreous cavity after injection in each experimental group ERGb wave amplitude decreased significantly after 7 days, the anterior chamber injection group and the control group did not change significantly. 5mg experiment eye ERG b-wave amplitude decreased significantly lower than the other three experimental and control groups. Optical microscopy results show that the corneal endothelial cells of the anterior chamber injection group irregular hexagonal structure to reduce cell loss more, the nucleus becomes longer, the nuclear membrane is discontinuous typical hexagonal structure is still visible. The other group is better than the anterior chamber injection group. Retinal tissue structure layers arranged in neat rows, structural integrity, were no obvious abnormalities. TEM results show, the experimental corneal endothelial cells of the anterior chamber injection group cytoplasmic organelles sparse, visible short and expansion of the endoplasmic reticulum, the connection obvious damage. The other set of experiments eyes are good in the anterior chamber injection group. 5mg group most of the experimental eye optic fiber cross-sectional vacuolization, mitochondrial swelling; rarely than the normal structure of synaptic connections, although some of neurotransmitter vesicles the postsynaptic membrane high MDF missing, the other group did not see the obvious exception. Clinical observations show that the testing of all patients with corneal endothelial cells showed no significant change, the difference was not statistically significant (p gt; 0.05). ERGb wave and peak did not change significantly, the difference was not statistically significant (p gt; 0.05). Macular edema was significantly reduced, the difference was statistically significant (p lt; 0.05). Retinal neovascularization significantly subsided significantly reduced leakage. Conclusion: The following intravitreal injection 5mg Avastin animal retina in the short term organizational structure and nerve conduction no significant impact. 5mg group cross-section of the optic nerve fibers vacuolization, swelling of mitochondria, suggesting that a single dose of intravitreal injection of Avastin is not easy too large, the interval is too short is not easy to reduce or avoid Avastin accumulation of toxic side effects produced on the retina. Clinical intravitreal injection of Avastin 1.25mg safe.
