Abstract


Objective: To investigate the vitreous cavity joint injection aids vitreous cutting surgical treatment of severe proliferative diabetic retinopathy difficulty of operative surgery and postoperative results. Methods: 65 eyes of 60 cases of proliferative diabetic retinopathy VI patients did not undergo preoperative retinal laser treatment, were randomly divided into A, group B groups, A group of 34 eyes with vitreous cavity joint injection group, preoperative 5 -7 days intravitreal injection the bevacizumab (bevacizemab) 1.25mg vitrectomy surgery intravitreal injection of triamcinolone acetonide (triamcinoloneacetonide TA) 1 mg. Group B, 31 eyes for the control group, the vitreous cavity is not injection, simple vitrectomy. After 1 week in January, March, using decimal visual acuity chart testing vision elastic intraocular pressure detection IOP, OCT observation macular edema, ophthalmic B-and slit lamp plus the 100D front mirror check fundus. Surgery, postoperative complications, operative difficulty (surgery time, and silicone oil tamponade), visual acuity, retinal anatomic reduction rate were analyzed. Results: 1 two groups of patients in the comparison of complications: A group of blood loss rate of 29%, new hole formation rate of 23%, plastic film rate of 8%, group B blood loss rate of 81%, a new hole formation rate of 87%, the plastic film rate of 49% in group A compared to group B intraoperative complication rate is significantly reduced, the two groups have a significant difference (P lt; 0.01). 2 Comparison of postoperative complications: A group of postoperative pupil membranous exudation rate of 8.8% the, recurrent vitreous blood rate of 5.9%, retinal neovascularization re-proliferative rate of 0%. Group B pupil membrane exudative rate 64.5% 19.4% rate of recurrence vitreous blood, the retinal neovascularization again proliferative rate of 9.7%. Postoperative pupil of Group A film exudative rate, recurrent vitreous hemorrhage rate, retinal neovascularization re-proliferative rate less than that in group B, the two groups have a significant difference (P lt; 0.05). Comparison of the two surgical time in group A mean operative time of 110 minutes, the B group, the average operation time was 140 minutes in group A than in group B shortened by 30 points. Silicone oil tamponade in 4 patients rate A set of silicone oil filling rate of 30%, 60% of the B group of the silicone oil filling rate, the two groups have a significant difference (P lt; 0.01). 5 postoperative visual comparison postoperative January, March BCVA change without statistical differences (P gt; 0.05). 6 retinal reattachment rate of A, B two groups retinal anatomic reduction rate of 100% and 98%, respectively, no significant differences (P gt; 0.05). Conclusion: The treatment method of visual acuity, retinal reattachment rate was no difference. The 2 vitreous cavity joint injection drug assisted vitrectomy surgical treatment of severe proliferative diabetic retinopathy, shorten the operation time, reduce operative difficulty and risk of surgery. 3 vitreous cavity injection drug assisted vitrectomy surgical treatment of severe proliferative diabetic retinopathy enables joint surgery, postoperative complications reduced.
