Short-term and Long-term Effects of Endoscopic Argon Plasma Coagulation for the Treatment of Barrett’s Esophagus: a Prospective 3-year Follow-up

Abstract


Objective To observe the Barrett's esophagus (BE) with endoscopic argon plasma coagulation therapy after surgery (APC) near-term and long-term efficacy and related factors of lesion recurrence . Methods Digestive Endoscopy Center of Tongji Hospital from August 2006 to March 2010 were stained magnification endoscopy and biopsy - confirmed 483 cases of BE patients ( male 292 cases , 191 cases of women ) . All patients in the endoscopic argon ion coagulation routinely given after proton pump inhibitor (PPI) therapy for 8 weeks , treatment to continue to give the PPI -demand control of symptoms in some patients after 8 weeks . After February , June , December , 18 months , 24 months , 30 months , 36 months review dyeing magnifying endoscopy to evaluate the efficacy and lesion recurrence . All patients relapse again endoscopic APC therapy . Results February review stained magnifying endoscopy , all patients with lesions completely disappeared and no recurrence of the 6 -month review of lesions review once every 6 months thereafter stained magnifying endoscopy to 36 months , found that lesions since 12 month relapse after the relapse rate was gradually increased to 36 months , the cumulative recurrence rate of 42.86% . All patients were followed up for 36 months, no serious complications and cancerous situation . APC treatment of patients relapse again line endoscopic lesions disappeared . The existence of significant differences , different general types stained magnifying BE relapse rate and lowest recurrence rate of the island , mixed the highest , tongue -type in between . Conclusion Endoscopic APC treatment of Barrett's esophagus the recent significantly related to effective and safe, but can not prevent the disease recurrence , the cumulative recurrence rate gradually increased with the extension of the follow-up time , recurrence and endoscopic BE typing . Therefore , the long-term follow-up of patients with Barrett's esophagus after endoscopic APC is very necessary, to discover recurrent lesions and to give appropriate treatment to improve the long-term prognosis of the disease .
